摘要
目的:检测肝门部胆管癌中凋亡抑制因子Survivin蛋白的表达,研究其与肝门部胆管癌的临床特征的关系,并探讨其与p53的相关性。方法:采用免疫组织化学方法检测86例肝门部胆管癌组织、10例癌旁组织(距肿瘤边缘约1.0cm)及10例正常胆管组织中Survivin蛋白和p53蛋白的表达,结合临床病理学资料进行统计学分析。结果:86例肝门部胆管癌组织中,Survivin蛋白阳性率为82.6%(71/86),p53蛋白阳性率为76.7%(66/86),Survivin与p53在癌旁组织和正常胆管组织中均呈阴性表达,与癌组织相比差异有统计学意义(P<0.05);Survivin表达与肝门部胆管癌病理分级、Bismuth分型、患者的性别及年龄和肿瘤大小均无关(P>0.05),但淋巴结转移阳性组Survivin蛋白表达水平明显高于淋巴结转移阴性组,差异有统计学意义(P<0.05);p53蛋白在肝门部胆管癌中与肿瘤组织病理分级、淋巴结有无转移相关(P<0.05),与其他病理学特征无关(P>0.05)。71例Survivin阳性表达者中同时表达p53阳性者58例,15例阴性表达者中表达p53阳性者8例,两者差异有统计学意义(P<0.05)。结论:肝门部胆管癌组织中Survivin和p53的表达显著高于癌旁组织和正常胆管组织,其在肝门部胆管癌的发展、转移过程中可能具有重要作用。
Objective:To investigate the expression of Survivin and its relationship with the clinical features of hilar cholangiocarcinoma and the expression of p53. Methods: The expressions of Survivin and p53 were detected in 86 specimens of hilar cholangiocarcinoma. 10 specimens of tissues near the carcinoma and 10 specimens of normal bile duct tissues by immunohistochemistry method.Laboratory data were analyzed with the related clinical-pathological data. Results, Survivin was expressed in 71 (82.6%) cases of 86 cases, p53 was expressed in 66 (76.7%) cases. Survivin and p53 were not detected in tissues near the carcinoma and normal bile duct tissues. The expression of Survivin in hilar cholangiocarcinoma had no association with age and gender(P 〉 0.05), tumor size (P 〉 0.05), the Bismuth type (P 〉 0.05), pathological grading (P〉 0.05),but the expression was closely related with lymph node metastasis (P〈0.05). The ex- pression of p53 in hilar cholangiocarcinoma was closely related with pathological grading and lymph node metastasis(P〈0.05), but the expression had no association with age and gender(P〉 0.05), tumor size(P〉 0.05), the Bismuth type(P〉 0.05). p53 was expressed in 58 cases of the 71 Survivin-positive cases, p53 was expressed only in 8 cases of the 15 Survivin-negative cases. The difference had statistical significance (P〈0.05). Conclusions: The expressions of Survivin and p53 in hilar cholangiocarcinoma were significantly higher than those in tissues near carcinoma and normal bile duct tissues.Joint action of Survivin and p53 might play an important role in the oncogenesis and metastasis of hilar cholangiocarcinoma.
