摘要
目的采用新生儿脐血血红蛋白Bart含量测定,分析广西恭城瑶族自治县瑶族和汉族新生儿α地中海贫血的发生率。方法采用常规血红蛋白电泳Bart定量测定2004例新生儿脐血,根据国内现行诊断标准情况进行比较分析。结果瑶族、汉族新生儿地中海贫血检出率分别为9·71%和7·72%,两者比较其差异有统计学意义(P<0·05)。瑶族与汉族男、女性新生儿α地中海贫血检出率的比较均无统计学意义(P>0·05);而瑶族男、女新生儿α地中海贫血检出率分别高于同期检测的汉族男、女性新生儿(P<0·05)。在检出四个类型的α地中海贫血中,以α1地中海贫血检出率最高,占6·69%,且瑶族新生儿α1地中海贫血检出率高于汉族新生儿(分别为7·39%和4·97%,P<0·05)。结论瑶族新生儿α地中海贫血发生率高于汉族新生儿;瑶族男、女新生儿α地中海贫血发生率均分别高于汉族男、女新生儿;在检出的α地中海贫血四个类型中,以α1地中海贫血发生率最高,且瑶族新生儿α1地中海贫血发生率高于汉族新生儿。
Objective To study the incidence of a thalassemia of Yao and Han ethnic neonates through measuring the hemoglobin （Hb） Bart （Fib-Bart）＇ s level in umbilical cord blood from those neonates in Gongcheng Yao ethnic autonomous county of Guangxi. Methods The Fib-Bart level was qualified in umbilial cord blood from 1421 Yao ethnic neonates （710 boys and 711 girls）and 583 Han neonates（296 boys and 287 girls） by using electrophoresis, and the positive rates of the α thalassemia between the two ethnic neonates were analysed and compared between the two ethnic neonates according to the domestic concern＇ s classification of α thalassemia. Results The positive rates of a thalassemia in Yao and Han ethnic neonates were 9. 71% and 7.72% （ P 〈 0. 05 ） respectively. The positive rates of the a thalassemia were not obviously differences between Yao boy and girl neonates （9.58% V. s 9. 85%, P 〉0. 05 ） ,and between Han boy and girl neonates （7.77% V. s 7.67% ,P 〉0. 05 ） ,the Yao ethnic boy or girl neonates was higher than those neonates of Han （ 〈 0. 05 ） , respectively. Another, it was foundthat the positive rate α1 thalassemia was the highest among the 4 types of α thalassemi in the Yao and Han ethnic neonates, and the positive rate of Yao ethnic neonates was higher than Han in this disorder （7.39% V. s 4.95 % ,P 〈 0. 05 ）. Conclusion It is suggested that the Yao neonates is higher than Han in the incidence of the α thalassemia, the Yao ethnic boy or girl was higher than those of the Han, the incidence α1 thalassemia was the highest among the 4 types of α thalassemia in the two ethnic neonates, and Yao ethnic neonates is higher than Han in the incidence of the α1 thalassemia in Gongcheng Yao ethnic county of Guangxi.
