摘要 目的观察妊娠期肝内胆汁淤积症(ICP)孕妇静脉血、羊水、脐血中胆酸、促红细胞生成素(EPO)变化及其相关性研究,探讨其临床意义。方法对52例ICP患者采用酶联免疫吸附试验(ELISA)和放免法(RIA)测定孕妇血、羊水、脐静脉血中胆酸及EPO水平;另选取46例正常足月妊娠孕妇作对照组;胎儿娩出时抽取脐动脉血进行血气分析以判定是否缺氧。结果ICP组孕妇血、脐静脉血及羊水中胆酸水平显著高于对照组;ICP组孕妇血EPO水平同对照组相比差异无统计学意义(P>0.05),ICP组妇女的胎儿脐静脉血及羊水中EPO含量均明显低于对照组胎儿的水平(均P<0.05),ICP组中缺氧胎儿的脐静脉血EPO水平明显低于未发生缺氧胎儿的水平,ICP组孕妇血、脐静脉血及羊水中胆酸与脐静脉血、羊水的EPO分别成负相关。结论ICP孕妇血清高胆酸水平对胎儿体内合成EPO有一定影响;胎儿EPO水平低下是其宫内缺氧的原因之一;检测羊水EPO可以作为产前监测ICP胎儿宫内缺氧的一项参考指标。 Objective To investigate the changes of erythropoietin and cholyglycine in amniotic fluid and umbilical blood in intrahepatic cholestasis of pregnancy( ICP) and their relations, and to explore their clinical value. Methods ELISA and radioimmunoas-say(RIA.)were used to examine the levels of erythropoietin and cholyglycine in serum,amniotic fluid and umbilical blood in 52 pregnant women with ICP(ICP group)and 46 normal pregnant women(control group). Umbilical cord arterial blood gas analysis of new-bern fetus was used to identify the fetal hypoxia. Results The concentrations of cholyglycine in amniotic fluid and umbilical blood were higher in ICP group than in control group. The blood erythropoietin had no significant difference between ICP group and control group(P 〉0.05). The concentrations of erythropoietin in amniotic fluid and umbilical blood were lower in ICP group than in control group(P 〈 0.05). The concentrations of erythropoietin in newborn fetus with hypoxia in ICP group were lower than that without hy-poxia. The cholyglycine in blood,amniotic fluid and umbilical blood in ICP group was negatively correlated to erythropoietin concentration. Conclusion The higher level of cholyglycine in ICP pregnant women would affect the synthesis of erythropoietin in new-bern fetus, and the lower level of erythropoietin may be a factor of fetal hypoxia. Erythropoietin from amniotic fluid and umbilical bled in ICP group can be used as an indicator for predicting fetal hypoxia.
