摘要
目的 探讨艾司洛尔复合硝酸甘油在显微镜下单鼻孔入路垂体瘤切除手术中控制性降压的效果及对血流动力学的影响。方法选择显微镜下单鼻孔入路垂体瘤切除手术ASA Ⅰ~Ⅱ级的患者40例,随机分为Ⅰ组硝酸甘油组,术中单独用硝酸甘油降压;Ⅱ组艾司洛尔、硝酸甘油复合组。两组患者均采用气管内插管全身麻醉。术中Ⅰ组泵注硝酸甘油,Ⅱ组泵注硝酸甘油和艾司洛尔,记录两组患者手术中心电图(ECG)、心率(HR)、平均动脉压(MAP)、中心静脉压(CVP)、二氧化碳分压(PETCO2)、血氧饱和度(SpO2)。结果 Ⅱ组在降压过程中硝酸甘油用量及输注速度均低于Ⅰ组;Ⅰ组达降压目标后HR明显加块,Ⅱ组降压后HR减慢;两组CVP、PET、CO2、SpO2均正常。结论 艾司洛尔复合硝酸甘油可以安全用于显微镜下单鼻孔入路垂体瘤切除手术。
Objective To evaluate the effect of esmolol and glyceryl trinitrate on controlled hypotension and hemodynamics used in single nostril transphenoidal approach pituitary tumor remove surgery. Methods Forty patients, ASA Ⅰ or Ⅱ ,undergoing the single nostril transphenoidal approach pituitary tumor remove surgery were randomly divided into two groups. Patients were infused with glyceryl trinitrate only to controlled hy- potension in group I and glyceryl trinitrate combined with esmolol were infused in group. All patients were received general anesthesia with tracheal intubation. ECG, HR, MAP, CVP,PET CO2 and SpO2 were recorded during the surgery. Results The consumption of glyceryl trinitrate were significant fewer and the glyceryl trinitrate infusion rate were significant slower in group Ⅱ than in group Ⅰ. HR were increased in group Ⅰ and decreased in group Ⅱ when reach the desired level of controlled hypotension. CVP, PETCO2 and SpO2 were remained on normal level constantly during the surgery. Conclusion Esmolol combined glyceryl trinitrate can be safety used in single nostril transphenoidal approach pituitary tumor remove surgery.
