80W KTP激光汽化术治疗大体积前列腺增生的临床应用 被引量:2

Clinical study of 80W KTP photoselective laser vaporization on the patients with large-volume BPH

摘要 目的探讨80W KTP激光在治疗大体积前列腺增生中的临床应用。方法采用回顾性研究,对156例前列腺增生>60ml的患者行80W KTP激光汽化术(PVP)治疗。术前观察前列腺体积、血清钠以及血红蛋白含量。术中记录出血量及手术时间。观察术后第1天血清钠、血红蛋白含量。术后留置尿管时间及术后住院时间。结果采用硬膜外麻醉,手术均获得成功,无一例中转开放手术。术前平均前列腺的体积为(88.9±27.1)ml,术中出血量(60.8±24.1)ml、手术时间(110.9±29.7)min。无一例需要输血。术后留置尿管时间(2.1±0.7)d。术前与术后血红蛋白含量、血清钠无明显变化。术后住院时间(3.2±1.1)d。结论PVP治疗大体积前列腺增生具有创伤小、并发症少、安全等优点,是治疗大体积前列腺增生的理想方法。 Objective To investigate the safety and efficacy of high-power potassium-titanyl-phosphate photoselective laser vaporization of the prostate (PVP) in patients with prostate volume greater than 60 ml. Methods A total of 156 patients with large-volume benign prostatic hyperplasia were treated with an 80W potassium-titanyl-phosphate (KTP) transurethral photoselective laser vaporization of the prostate from October 2004 to December 2005. The preoperative evaluation including prostate volume, serum sodium and serum haemoglobin, the operative time and the bleeding volume were recorded during the PVP operation. And postoperative serum sodium, serum haemoglobin, average catheterization time and length of hospitalization were recorded after the operation. Results 156 cases were successfully operated under epidural anesthesia.No one was changed to open operation. The mean preoperative prostate volume was(88.9±27.1)ml. The mean bleeding volume was (60.8±24.1)ml. The mean operative time was (110.9±29.7)minutes. No transfusion was required.The mean indwelling catheterization was (2.1±0.7) days. The serum sodium and serum haemoglobin level did not change significantly. The length of stay in hospital was (3.2±1.1) days. Conclusion Photoselective laser vaporization of the prostate is a safe and effective therapy for large-volume benign prostatic hyperplasia.
