摘要
目的观察神经生长因子(nerve growth factor,NGF)滴眼液对兔眼准分子激光原位角膜磨镶术(laser in situ keratomileusis,LASIK)后角膜神经修复的影响。方法35只中华大耳白兔双眼接受近视性LASIK术,术后左眼用配置的100·g/mlNGF滴眼液,右眼滴平衡液(balanced salt solution,BSS)作为对照,1滴/次,3次/d。分别于术后第1天、第3天、第1周、第2周、第1个月、第3个月、第6个月各取5只兔,在观察双眼前后节反应后处死兔,摘除双侧角膜,进行角膜神经氯化金染色。光镜下观察LASIK术后角膜神经修复过程,分别比较角膜上皮、浅基质层,深基质层NGF组与BSS组LASIK术后不同时期角膜神经的数量。结果LASIK术后角膜深基质层、角膜瓣连接处的上皮下和浅基质层神经未受损,瓣切削处上皮下和浅基质层神经消失。术后不同部位的角膜神经再生程度不同,术后第6个月周边部角膜神经形态已接近正常,角膜中央仍无神经分布。NGF组与BSS组术后各时间点角膜上皮、浅基质层神经数量比较,NGF组大于BSS组,差异有显著性(P<0.05);两组术后深基质层神经数量比较,NGF组大于BSS组,差异也有显著性(P<0.05)。结论NGF滴眼液对LASIK术后不同部位、不同时期角膜神经的修复有明显促进作用。
Objective To investigate the effect of nerve growth factor (NGF) eye drops on rabbit corneal nerve reinnervation after laser in situ keratomileusis. Methods Laser in situ keratomileusis for myopia was performed on both eyes of 35 rabbits and then 100 μg/ml NGF eye drops were used on the left eyes and a balanced salt solution (BSS) was used on the right eyes, one drop, 3 times a day. After the anterior and posterior segments of the eyeball were examined, the rabbits were sacrificed l day, 3 days, 1 week, 2 weeks, 1 month, 3 months, and 6 months postoperatively and the corneas were removed for golden chloride staining. The effect of NGF eye drops on corneal nerve regeneration was observed and compared by counting the number of regenerating corneal nerves both in the epithelium and in both the shallow and deep stromal layers at the different time periods after surgery. Results Nerve fibers in the deep and shallow stroma and the subepithelium located at the hinge of the corneal flap were not damaged. However, the subepithelial nerve plexus and shallow stromal nerves outside the hinge area of the corneal flap were absent after LASIK. Budding nerve fibers grew toward the shallow stroma and regenerating nerve trunks in the deep stroma could be seen on the 3rd day after LASIK. Some regenerating nerves were seen in the subepitbelial nerve plexus and the shallow stroma within a 1-month period after LASIK. New nerves from the deep stroma formed networks and approached the shallow stroma at 2 months. Three months later, regenerated nerves of the deep stroma had connected into the subepitbelial nerve plexus. The nerves in the peripheral cornea showed complete recovery, but no nerve fibers were seen in the central cornea at 6 months. There was a significant difference in the number of regenerating nerves in the corneal epithelium and the shallow and deep stromal layers when the NGF group and BSS group were compared at different periods after surgery (P〈0.05).
