摘要
目的探讨成人髋关节发育不良全髋关节置换术前髋臼评估过程中三维CT重建技术的方法及临床应用价值。方法2003年1月~2007年2月,对13例成人髋关节发育不良患者进行螺旋CT扫描,通过表面遮盖和多平面重建观察髋臼的空间位置和髋臼壁骨质情况。结果髋臼变浅变小,髋臼外上方普遍存在不同程度的骨缺损,髋臼内侧壁和后壁有一定量的骨贮备。部分髋关节骨赘增生严重,除了覆盖髋臼使髋臼变浅外,髋臼边缘的骨赘使髋臼的前倾角变小。结论螺旋CT表面遮盖结合多平面重建是评估髋臼形态学的有效手段,对成人髋关节发育不良术前假体的选择和术中髋臼的重建具有较高的参考价值。
Objective To explore the method and application value of three-dimensional reconstructive technique of spiral CT in the evaluation of acetabulum anatomy with developmental dysplasia of the hip before total hip artroplasty. Methods 13 cases with developmental dysplasia of the hips were examined with spiral CT, surface shaded display(SSD) and mulfiplanar reconstruction(MPR) to observe the spacial location of acetabula and sclerofin of the acetabular wall, so as to analyze their morphological changes. Results All acetabula become smaller and shallow. Superior-lateral walls of acebabula have various extent bony deficiency, but medial and posterior walls of acebabula have some-quantity bone-stock. Osteophytes in some acetabula accrementition seriously and make acetabula more shallow and osteophytes in the anterior rim of acetabula make it less anteversion. Conclusion SSD and MPR of spiral CT are effective methods in evaluating the acetabulum morphology, and have strong implication to prosthesis selection pre-operation and acetabulum reconstruction in-operation.
