摘要目的:全面了解军干病房医院感染分布趋势,为评价和完善监控系统提供依据。方法:采用描述性研究与定性访谈相结合的方法,对2001-2005年军干病房医院感染资料进行调查。结果:2001-2005年军干病房逐年医院感染率依次为7.62%、8.20%、7.24%、7.24%、9.91%(P〉0.05);死亡率为36.51%、22.34%、20.45%、12.87%、13.39%(P=0.00);医院感染病例病原学送检率为29.73%、27.78%、28.28%、37.93%、46.79%(P=0.00);医院感染病原菌构成比革兰阴性杆菌为9.52%、11.11%、45.00%、48.64%、43.10%;真菌为57.14%、51.85%、35.00%、21.62%、20.69%。历年医院感染部位居第1和第2位的是下呼吸道和上呼吸道。结论:军干病房医院感染率和死亡率均高于全院平均水平,其中,医院感染率逐年差异无显著意义,死亡率有逐年下降的趋势。感染部位以呼吸道为主。医院感染病原学送检率和送检阳性率、革兰阴性菌感染有逐年上升的趋势。
Objective:To study the distribution and secular trend of nosocomial infection(NI) in military cadre's ward and to provide basic data for evaluation and perfection monitoring system. Methods.. The NI data of a military cadre's ward of a hospital from 2001 to 2005 were analyzed with the method of descriptive study combined with qualitative interview study. Results:NI incidences of this cadre's ward in five years were 7.62%, 8. 20%, 7.24%, 7. 24% and 9. 91% (P=0.07), and the death rates were 36.51%, 22. 34%, 20. 45%, 12. 87% and 13. 39% from 2001 to 2005(P=0. 00), respectively. The rates of NI pathogenic bacteria tested were 29. 73%, 27. 78%, 28. 28%, 37.93% and 46.79% (P=0. 00), respectively. The constituent ratios of gram negative bacilli of NI pathogenic bacteria were 9. 52%, 11.11%, 45. 00%, 48. 64% and 43. 10% and the constituent ratio of fungus were 57.14%, 51.85%, 35. 00%, 21.62% and 20. 69%, respectively. NI occurred most in lower respiratory tract and upper respiratory tract. Conclusion: The incidence of NI and the case fatality rate of this military cadre's ward are higher than those of the whole hospital. The difference of the incidence of NI among five years is not significant, but the descending secular trend of case fatality rate is significant year by year. The most common infection position is respiratory tract. The rate of NI pathogenic bacteria tested and the positive rate ascend and the constituent ratio of gram-negative bacilli of NI pathogenic bacteria is higher and higher year by year.
