摘要
目的探讨外伤性白内障手术治疗时机，术式选择及其效果。方法总结外伤性白内障81例（81眼）。对其中76例行白内障一期和二期摘出、人工晶状体一期或二期植入术（5眼因眼球破裂伤严重，放弃人工晶状体植入）。经随访3～12个月，对其效果进行分析。结果一期白内障摘出联合后房型人工晶状体植入46眼，术后视力在0．06～0．8之间；其中7眼0．06～0．08，11眼0．1～0．25，12眼0．3～0．4，16眼0．5～0．8，其中0．3～0．8共28眼。占一期人工晶状体植入60．87％，二期自内障摘出联合人工晶状体植入和单纯二期人工晶状体植入术30眼，术后视力在0．2～1．0之间，其中0．3～1．0者28眼。占二期人工晶状体植入93．33％。二期人工晶状体植入组的并发症较一期手术组少而轻。结论二期人工晶状体植入术效果较好，对于后囊裂开，眼后段情况不明者，不要一期植入人工晶状体。
Objective To investigate the operation chance of traumatic cataract. Methods 81 cases of traumatic cataract admitted our hospital from July 2003 to July 2006 were summarized, choose stage Ⅰ and stage Ⅱ extraction of cataract,artificial stage Ⅰ and Ⅱ lens implantation,a total of 76 eyes follow up 3 - 12 months and the effect were analyzed. Results 46 cases stage Ⅰ cataract extraction combine with posterior chamber artificial lens implantation,vision from 0.06 to 0.8 after operation, among them 39 eyes （84.78%） above 0.1,28 eyes（61% ） ,from 0.3 to 0.8, 30 eyes stage Ⅱ extraction of cataract combine with artificial lens implantation and simple stage Ⅱ artificial lens implantation and simple stage Ⅱ artificial lens implantation, vision improved from 0.2 to 1.0 among them,28 eyes improved from 0.3 to 1.0 dominated 93% ,in stage Ⅱ artificial lens implantation group, the compli cation was less and not so severe compare with stage Ⅰ group. Conclusions stage Ⅱ artificial lens implantation had rather good effect for those patients with rupture of posterior capsule or posterior eye condition of unclear do not choose stage Ⅰ implantation of artificiallesn.
