声学定量彩色室壁运动技术评价正常人左室壁收缩功能 被引量:7

Evaluation of Normal Regional Left Ventricular Contracation Using Acoustic Quantification Color Kinesis

摘要 彩色室壁运动(colorcinesis,CK)分析技术是在声学定量(AQ)基础上新发展的一种超声组织定征方法,它能够实时显示心动周期不同相心内膜的位移,并辅以不同的彩色进行编码。本研究观察了13例健康人左室壁收缩运动变化。结果显示:从左室整体收缩运动幅度来看室间隔运动幅度最低(P值<0.05),其余各节段室壁运动无显著差异;左室收缩末期各节段室壁心内膜位移四腔切面:侧壁基底段11.3±1.1mm、中段11.2±1.4mm、心尖段10.3±1.1mm、室间隔基底段10.2±1.2mm、中段10.2±1.8mm、心尖段9.5±1.5mm;二腔切面:左室前壁基底段113±1.8mm、中段10±1.1mm、心尖段10±2.5mm、下壁基底段10.1±1.7mm、中段10±1.1mm、心尖段10±1.2mm;左室乳头肌水平:前壁11.4±1.3mm、侧壁11.2±1.5mm、后壁10.4±2.0mm、下壁10.9±1.6mm、前间隔10.7±1.0mm、后间隔10.2±1.0mm。初步研究结果表明,CK技术可以定量分析室壁节段运动。 Acousticquantifcation color kinesis(AQCK)is anew technique for the echocardiographic assessment of regional left ventricular(LV)wall motion based on acoustic quantification technique.The aim of this issue was to describe the normal patterns of regional LV contraction using AQCK.Apical 4- chamber LV views were obtained in13 normal subjects(mean age33±2.5),apical2-chamber views in 8 normal subjects(mean age32±2.2)and short-axis level of LV in 9 normal subjects(mean age33.1±2.3).Each systolic color map was then divided into 30° wedgeshape segment through the central point of mitral valve annular at the apical views.Six segments was divided at papillary muscles level.In each segment,the position shift of each color was measured respectively in late phase of systole.Results from the view Of LV contractive amplitude,the motion amplitude of interventricular septum is the lowest,other segments are no significantly different the anterior:basal segment was 11.3±1.8(mm),middle segment 11±2.7(mm),apical segment10.1±2.5(mm);the inferior:basal segment10.,1±1.7(mm),middle segment10±1.1(mm),apical segment10±1.2(mm);the lateral:basal segment11.3±1.1(mm), middle segment11.2±1.4(mm),apical segment 10.3±1.6(mm);interventricular septum basal segment 10.2±1.2(mm),middle segment10.2±1.8(mm),apical segment 9.5±1.5(mm);at papillary muscles levelanterior wall11.4±1.3(mm),lateral wall11.2±1.8(mm),posterior wall10.4±2.0(mm), inferior wall10.9±1.6(mm),anteseptum10.7±1.0(mm),postseptum10.2±1.0(mm).Conclusion: quantitative analysis of AQCK provides accurate and highly reproducible assessment of LV regional wall motion.The normal pattems of contraction hereby described may be used as a reference for objective interpretation of ergional wall motion abnormalities.
