摘要
目的了解新生儿缺氧缺血性脑病(HIE)血淋巴细胞L选择素、细胞间黏附因子-1(ICAM-1)水平的变化,探讨其在HIE的意义。方法采用流式细胞仪检测35例HIE患儿、20例出生时窒息但无HIE患儿生后第1天及21名足月健康新生儿脐血淋巴细胞表面L选择素和ICAM-1的水平。结果HIE患儿生后第1天血液淋巴细胞L选择素水平与窒息组、健康对照组比较差异无统计学意义,健康对照组、窒息组、轻度HIE组与中重度HIE组血淋巴细胞表面ICAM-1的水平比较差异均有统计学意义,分别为(5.64±3.54)%、(6.94±3.54)%、(8.52±5.19)%及(14.50±3.10)%,HIE程度越重,血淋巴细胞ICAM-1越高。结论血淋巴细胞表面L选择素不能反应窒息患儿是否存在HIE及其严重程度,ICAM-1可能参与缺氧缺血性脑损伤的发病机制,血淋巴细胞表面ICAM-1可以反应窒息患儿是否存在缺氧缺血性脑损伤及其严重程度。
Objective To study the dynamic changes and significance of lymphocyte L-selectin and ICAM-1 levels in the first 24 hours of life in newborns with hypoxic-ischemic encephalopathy (HIE). Methods Thirty-five newborns diagnosed with HIE and 20 newborns with asphyxia without HIE were ineluded in this study, 21 healthy newborns were selected as control group. Their blood lymphocyte L-selectin and ICAM-1 in the first 24 hours of life were compared with that of cord blood in healthy newborns by flow cytometry. Results The level of lymphocyte L-selectin in the first 24 hours of life in HIE group was similar to that in control group and asphyxia group. The level of blood lymphocyte ICAM-1 in newborns with severe HIE was significantly higher than that in control group, mild HIE group and asphyxia group (14.5±13.1)% vs (5.64±3.54)%, (8.52 ±5.19)% and (6.94±3.54)%, F= 4. 426, P 〈 0. 007. The more severe the HIE was, the higher lymphocyte ICAM-1. Conclusions There are no obvious changes of lymphocyte L-selectin level in the first 24 hours of life in newborns in all stud- ied groups. The increased level of ICAM-1 may be involved in the pathogenesis of HIE in the first 24 hours of life of newborns and may represent the severity of HIE.
