摘要
目的:应用超声导向肝穿活检,明确肝内占位病变性质,选择最佳治疗手段。材料和方法:使用Aloka-SSD-250及AL-310B超仪,35MHz线阵穿刺探头及18或22号带针芯长针,对284例性质不明的肝内占位病变进行实时超声引导下肝穿针吸细胞学检查,并与手术病理结果对照,回顾性验证肝穿细胞学诊断准确性,分析了小肝癌与炎性假瘤声像图特征。结果:本组284例,肝穿细胞学诊断恶性肿瘤符合率906%,良性肿瘤诊断符合率947%,小肝癌诊断符合率918%。结论:本法安全简便,快速准确,为肝内良恶性占位诊断及鉴别诊断的首选方法。
Purpose:Ultrasound -guided needle liver biopsy was used to distinguish the character of hepatic space occupying lesions so as to select the best treatment methed. Materials and Methods: Real time Ultrasound-guided needle biopsy was performed in 284 cases with hepatic space occupying lesions by using Aloka-SSD-250 and AL-310 mode ultrasound systems and 3.5MHz Puncture linear scan head and 18 or 20 number needles with the wick. The diagnostic accuracy of needle liver biopsy was verified and compared with the operative-pathologic result. The features of acoustic image of microhepatic cancer and hepatic inflammatory pseudotumor were analyzed. Results: In malignant tumor, the diagnostic accuracy of ultrasound-guided needle liver biopsy was 90.6%. 94.7% in benign tumor. For small hepatic carcinoma (≤5cm), the accuracy was 91.8%. Conclusing: Because of its safe, simple, quick and accurate advantages, we belived that, ultrasound-guided needle liver biopsy was the first method of choice in the diagnosis and differential diagnosis of space occupying lesions, either malignant or benign.
