摘要
目的研究非骨水泥型亚洲髋关节系统的近期临床疗效。方法测量全髋关节置换50例疗效,随访方法:①Harris髋评分系统;②影像学观察采用Engh评分系统来评估股骨侧假体的骨长入及假体稳定性,Wixsen方法来评估股骨柄假体在股骨髓腔内的匹配与充填。结果有30例获得40个月的随访,术前的Harris髋评分平均为(39±14.3)分(20~62分),术后评分为(93.3±4.4)分(83~100分),Engh评分得分为(25.6±3.9)分(17.9~28分),所有患者都有股骨距萎缩,提示所有股骨柄都已发生骨结合,而对照组(施乐辉髋关节Synergy系统)30例Engh评分平均为(18±3.5)分(10.9~26分),差别有统计学意义(P〈0.01)。非骨水泥型亚洲髋Wixsen评分近端1/3为(68.8±8.5)%(53.6%-80%)、中1/3为(88.7±11.8)%(79%~100%)、远端1/3为(84.2±11.6)%(64.4%~100%),股骨柄近端1/3与股骨髓腔内的匹配与充填非骨水泥型亚洲髋高于对照组(P〈0.05)。结论非骨水泥型亚洲髋关节系统股骨柄与股骨干骺端匹配较佳,两者之间有较高的骨结合率,在中国人群中的应用,其近期的临床随访疗效较好。
Objective To study the short - term effects of the eementless Asian Hip System designed according to femur measurement in the Asian populations. Methods Fifty patients underwent the total hip arthroplasty with Asian Hip System through modified lateral approach in the standard lateral decubitns position. The clinical results were evaluated by Harris scoring system. The bone integration stem stability and the fit - fill of the stem were tested by the method described by Engh and Wixsen, respectively. The data obtained were compared with that of another group of 30 cemenfless hips in the patients who were matched in sex, age, disease and follow - up period. Results Thirty patients were followed up for 40 months (22-50 months). The preoperative and postoperative average Harris scores were 39 ± 14.3 (20- 62), 93.3 ± 4.4 (83- 100) respectively. The Engh score was 25.6 ± 3.9 ( 17.9 - 28) in the group of agian Hip, and 18 ± 3.5 (10.9-26) in the control group. There was significant difference (P〈0.01). In the group of Asian Hip, all patients had calcar atrophy indicative of the occurrence of bone integration around the stems. Wixsen score in the proximal, intermediate and distal part in the group of Asian Hip were (68.8 ± 8.5) % (53.6% -80% ), (88.7 ± 11.8) % (79% - 100% ), (84.2 ± 11.6) % (64.4 % -100 % ), respectively. The fit - fill of the stem in the proximal part in the group of Asian Hip was better than that in the control group. (P 〈 0.05) Conclusion The cementless Asian Hip System designed according to femur measurement in the Asian populations has good match with the metaphyses of femur. The high incidence of bone integration happens in the interfaces between the stems and bone. The good short - term effects of the cementless Asian Hip System have been achieved in the Asian populations.
