慢性肾脏病患者左心室心肌质量指数与血清甲状旁腺激素、钙磷乘积的关系 被引量:9

摘要 目的研究中晚期慢性肾脏疾病(CKD)患者左心室心肌质量指数(LVMI)与甲状旁腺激素(PTH)、钙磷乘积之间的关系。方法选择81例CKD35期患者作为试验组,30例正常人作为对照组,作心脏超声检查,测量左心室舒张末内径(LVDd)、左心室收缩末期内径(LVDs)、室间隔舒张末厚度(IVS)、左心室后壁舒张末厚度(PWTH)、左心室射血分数(EF),舒张早期和舒张晚期二尖瓣口最大血流速度之比(E/A),用公式计算左心室心肌质量指数(LVMI),并同期测定血清PTH、钙磷乘积,然后统计分析其相关关系。结果①试验组患者超声心动图结果显示,38.27%存在LVDd增大(49.33±6.78)mm,71.61%存在IVS增厚(12.57±2.58)mm,49.38%存在PWTH增厚(11.91±2.33)mm,30.51%存在EF下降(0.58±0.29)%,41.76%存在E/A下降(0.74±2.99),有93.90%出现左心室肥大(LVH)[LVMI(187.06±73.85)g/m^2]。②CKD3-5期患者LVMI改变与PTH、钙磷乘积呈正相关,且PTH与LVMI独立相关(P〈0.05)。结论CRF患者心血管疾病的发生率很高,心脏结构功能的变化与PTH、钙磷乘积密切相关,且PTH是LVMI的独立危险因素。 Objective To study the correlation between the left ventricular mass index (LVMI) and parathyroid hormone (PTH), calcium-phosphorus product in chronic kidney disease (CKD) patients. Methods 81 patients with CKD and 30 healthy subjects served as control were enrolled in this study. LVDd,LVDs,IVS,PWTH,EF and E/A were measured by ultrasonic cardiogram. LVMI were calculated by the equation. Then the relationship between LVMI and PTH, calcium-phosphorus product were analyzed. Results The echocardiographic results of 81 patients indicated that there were 93.90% of LVH (LVMI 187.06±73.85g/m^2). Among them, 38.27% had enlarged LVDd(49.33±6.78) mm, 71.61% had increased IVS(12.57±2.58)mm, 49.38% had increased PWTH(11. 91±2.33)mm, 30.51% had decreased EF(0.58±0. 29)%, 41.76% had decreased E/A(0. 74±2. 99),and centrality reconstitution were 8. 54%. Statistical analysis showed a positive correlation between LVMI and PTH,calcium-phosphorus product. Conclusion There is a high incidence of cardiovascular disease in CRF patients. A close correlation exists between LVMI and disorders of phosphorus and calcium metabolism and between LVMI and PTH .
