摘要
目的探讨以胃癌为第一诊断的多原发癌的临床特点及漏诊原因诊治体会。方法对我院2002-2007年以来有完整资料的以胃癌为首诊的9例同时性多原发癌进行回顾性分析。结果9例同时性多原发癌仅有2例在手术前确诊,手术前确诊率为22.2%,其余7例手术前漏诊,漏诊率为77.8%。结论尽可能完善的术前检查和仔细的术中探查是发现第二原发癌的重要手段,手术时应根据不同病理类型分别处理。
Objective To explore the clinical features and reasons for misdiagnosis of multiple primary cancer as gastric cancer for the first diagnosis. Methods From January 2002 to October 2007, the clinical data of 9 patients with synchronous carcinoma as gastric cancer for the first diagnosis were analyzed retrospectively. Results Only two cases were diagnosed correctly before operation in nine cases of synchronous carcinoma. Preoperative correct diagnosis rate was 22.2%, and the residual 7 cases were misdiagnosised before operation and misdiagnosed rate was 77.8%. Conclusion The perfect preoperative examination and careful exploration during operation are important means to discover the second primary cancer. Operative management should depend on different pathological typing.
