摘要
目的分析仅吻合单根动脉指尖再植存活的临床现象,探讨其血流动力学机制。方法建立流体力学试验模型,确立仅吻合单根动脉指尖再植存活的血流动力学机制,并对36指仅吻合单根动脉存活的指尖再植进行总结。结果实验证实该流体力学试验模型的成立。临床上36指指尖再植,存活34指,失败2指,成活率为94.5%。再植术后2周彩色超声检查证实:仅吻合单根动脉吻合口的近端侧支存在回流。结论仅吻合单根动脉指尖再植存活机制主要是动脉吻合口近端的侧支回流。
Objective To observe the clinical featttres of fingertip survival following replantation by anastomosing one artery and investigate the blood flow dynamics. Methods Thirty-six cases of fingertip replantation that survived after single artery anastomosis were involved in the study. The blood flow dynamics was examined by applying hydrodynamics test model along with color ultrasonic examination. Results The hydrodynamics test model has been shown to fit the blood flow dynamics of fingertip replantation. Thirty-four out of the 36 replanted fingertips survived. Two fingertips were lost. The overall survival rate was 94.5 %. Color ultrasonic examination done at 2 weeks after replantation confirmed the existence of flux in communicating branches proximal to the arterial anastomosis site. Conclusion The mechanism of survival for fingertip replantation by single artery anastomosis mostly depends on the flux in commtmicating branches proximal to the arterial anastomosis site.
