摘要
目的探讨老年胃疾病患者胃黏膜病理改变过程中，血清胃蛋白酶原（PG）I、Ⅱ的变化规律。方法选择我院消化内镜中心行胃镜检查老年患者276例，按病理诊断标准分为4组，非萎缩性胃炎组（对照组）34例，慢性萎缩性胃炎组（胃炎组）71例，消化性溃疡组（溃疡组）105例，胃癌组66例。用免疫放射法（IRMA）测定患者血清PGI及PGⅡ，并计算PGI／PGⅡ的比值（PGR）。结果对照组PGI、PGⅡ和PGR分别为（149．26±62．45）μg／L、（15．66±8．97）μg／L和10．62±3．30，慢性萎缩性胃炎组PGI[（119．32±41．87）μg／L]、PGR（7．19±3．02）较对照组降低（P〈0．05），胃癌组PGI[（95．39±22．80）μg／L]、PGR（5．86±3．87）较对照组明显降低（P〈O．01）；溃疡组PGI[（175．29±33．69）μg／L]、PGⅡ[（21．81±8．91）μg／L]较对照组升高（P〈0．05）。结论血清PGI、PGⅡ含量的变化及PGR对癌前状态和早期胃癌的诊断具有重要的临床意义，当血清PGI及PGR严重降低时，应尽早进行胃镜检查，以明确诊断。
Objective To investigate the serum pepsinogen level changes in elderly patients with gastric mucosal diseases. Methods A total of 276 elderly patients having the gastroscopy check in Beijing Friendship Hospital were enrolled in the study, and divided into 4 groups： non-atrophic gastritis group（34 cases）; chronic atrophic gastritis group（71 cases）; peptic ulcer group（105 cases）; gastric cancer group（66 cases） according to pathological diagnosis, The serum PG I and PG Ⅱ levels were determined by immunoradiometric assay （IRMA）. The ratio of PG I/PG Ⅱ （PGR） was calculated. Results The serum levels of PG I , PGⅡ, and PGR in the non- atrophic gastritis group （control group） ： （149. 26 ±62.45）μg/L, （ 15.66 ± 8. 97 ） μg/L, 10. 62 ± 3.30 respectively. The serum levels of PG I [（ 119.32 ± 41.87） μg/L] and PGR （ 7. 19± 3.02 ） in the chronic atrophic gastritis patients versus control group were obviously lower （all P 〈 0. 05）. The serum levels of PG I [（95.39±22.80）μg/L] and PGR（5.86 ± 3.87） in the gastric cancer patients versus control group were obviously lower（all P〈0.01 ）. The serum levels of PG I [（ 175.29 ± 33. 69） μg/L], PG I] [（21.81 ± 8.91 ）μg/L] in the peptic ulcer patients were obviously higher（all P〈 0. 05）than in non-atrophic gastritis group. Conclusions The low levels of serum PG I and PGR may indicate a severe gastric disease（such as the precancerous condition and the early gastric cancer）. If a patient has abnormal serum pepsinogen level, he or she should has a gastroscopy check as soon as possible.
