摘要
目的分析影响老年人PICC管留置时间的因素,探讨如何延长PICC置管时间和保障老年患者输液治疗的护理措施。方法356例需留置PICC管的60岁以上老年人,按规程进行PICC置管、输液和日常护理,详细记录病例资料,进行统计分析。结果353例置管成功占98.1%,平均留置时间（57.9±27.0）d;提前拔管43例占12.2%,拔管原因有堵管、静脉炎、感染或疑似感染。结论强化置管及使用操作者的护理技能培训,严格操作规程,可以减少静脉炎、堵管和感染并发症,延长PICC留置时间,保证治疗完成,减少患者痛苦。
Objective To analyze the factors influencing the retention time of peripherally inserted central catheter（PICC）,to explore how to prolong the retention time of PICC,and to suggest the improved nursing measures for ensuring the transfusion therapy in elderly.Methods Three hundred and fifty-six elderly patients requiring long-term intravenous treatment entered the study.The catheterization,transfusion and daily nursing were carried out according to the rules.Clinical data was recorded for analysis in detail.Results Among the 353 cases,the success rate of PICC catheter was 98.1%.The mean duration was（57.9±27.0） days after insertion.Forty-three（12.2%） PICCs were removed when the treatment were not completed.The reasons of removing catheter abnormally included occlusion,phlebitis,infection or suspicious infection.Conclusions PICC is a simple and safe device for elderly patients who required long-term intravenous therapy.It is necessary to strengthen the catheterization,use the nursing skill training for operator and strict the operation rules,thereby reducing the complications of occlusion,phlebitis and infection,prolonging the retention time of PICC,ensuring the completion of treatment and lightening the patients＇ pain.
