患者术中回收血与麻醉前外周血红细胞携氧及供氧能力和生存能力的比较 被引量:7

Oxygen carrying and delivering capacity and survival ability of erythrocytes salvaged during operation

摘要 目的比较患者术中回收血与麻醉前外周静脉血红细胞携氧及供氧能力和生存能力。方法择期手术患者30例,性别不限,ASAⅡ或Ⅲ级,年龄30~64岁,体重55—65kg,术中行自体血液回收。于麻醉前经外周静脉采血2ml(静脉血),于回输血液时经连接输血器的三通取血2ml(回收血),测定血液pH、氧分压(PO2)及血氧饱和度,计算校正的P50(使血红蛋白氧饱和度达50%时的PO2),采用酶联免疫法测定红细胞2,3-二磷酸甘油酸(2,3-DPG)浓度,采用化学法测定红细胞丙酮酸激酶(PK)活性。结果与静脉血比较,回收血P50、红细胞2,3-DPG浓度和PK活性差异均无统计学意义(P〉0.05)。与正常值比较,回收血和静脉血P50差异无统计学意义(P〉0.05)。结论自体血回收对红细胞携氧和供氧能力及生存能力无影响。 Objective To determine the oxygen carrying and delivering capacity and survival ability of erythrocytes salvaged during operation. Methods Thirty ASA Ⅱ or Ⅲ patients of both sexes aged 30-64 yr weighing 55-65 kg undergoing various surgical procedures during which blood salvage was performed were enrolled in this study. ZITI 2000 cell saver system (Beijing Fine Medical Instrument Co) was used for intraoperative blood salvage. Blood samples were taken from peripheral vein before anesthesia and from salvaged blood for determination of pit, PO2 and SO2. The P50 was calculated. The 2,3-DPG concentration and pyruvate kinase activity in RBCs were detected. Results There was no significant difference in Ps0, 2,3-DPG concentration and pyruvate kinase activity in RBCs between salvaged and peripheral blood. Conclusion Intraoperative blood salvage has no significant effect on O2 carrying and delivering capacity and survival ability of RBCs.
