摘要
目的分析膝关节麦氏试验阳性患者的病因和发病机制,探讨其在膝关节镜下的治疗方法和效果。方法该组麦氏试验阳性患者共126例,其中男性71例,女性55例,年龄14～73岁,平均40.2岁;病程4d～10余年。患者均表现有不同程度的膝关节疾患的症状或体征。对该组患者进行膝关节镜下诊断和治疗,分别于术前及术后3个月行Lysholm膝关节评分系统评定关节功能,用配对t检验比较患者治疗前后Lysholm膝关节评分。结果该组126例患者,半月板损伤(包括盘状半月板,下同)67例,关节软骨损伤21例,滑膜皱襞综合征19例,髌下脂肪垫卡压10例,关节内游离体2例,骨关节炎2例,滑膜炎(包括创伤性和色素沉着绒毛结节性滑膜炎)4例,前交叉韧带损伤1例;患者治疗3个月后Lysholm膝关节评分较治疗前明显提高,差异有显著性(P<0.001)。结论关节镜检查对于膝关节麦氏试验阳性患者是一种理想的诊断方法,进行相应的关节镜下的治疗可以取得良好的治疗效果。
[Objective] To figure out the causes and mechanism of the patients with positive McMurray＇s test and then study the methods and effects of arthroscope assisted treatment. [Methods] 126 cases with positive McMurray＇ s test （71 males and 55 females; an average age of 40.2 years ranging from 14 to 73 years; disease duration ranging from 4 days to 10 years; with some clinical symptoms and signs of a disease of the knee） had the arthroscope assisted diagnosis and treatment. The conditions of knee joints were assessed before and 3 months after treatment according to Lysholm＇s function scoring and then were compared according to paired t-test. [Results] In the 126 cases, there were 67 cases of meniscus injury （including discoid meniscus）, 21 cases of joint cartilage injury, 19 cases of synovial plica syndrome, 10 cases of Hoffa＇s disease, 2 cases of loose body, 2 cases of osteoarthritis, 4 cases of synovitis（including traumatic and pigmented villonodular synovitis） and 1 case of anterior cruciate ligament injury. The Lysholm＇s function score 3 month after treatment is significantly higher than that before treatment （P 〈0.001）. [Conclusion] Arthroscopic examination was a satisfactory method for diagnosis of patients with positive McMurray＇s test. Good effect can be expected after arthroscope assisted treatment.
