^(99)Tc^m-MDP bone scintigraphic characteristics and diagnosis of osteofibrous dysplasia

摘要 目的总结骨纤维结构不良症(OFD)99Tcm-MDP骨显像图的特点,并与原发性骨肿瘤和转移性骨肿瘤进行比较鉴别,以提高OFD的临床诊断率。方法临床诊断OFD患者97例,均接受常规99Tcm-MDP全身骨显像并经手术病理证实。对99Tcm-MDP骨显像进行阅片,统计异常表现的位置、分布和图像特点。结果单骨型OFD有86例,均累及四肢骨,其中股骨最多见,44例(51.14%);其次是胫骨,24例(27.90%)。多骨型11例,骨病灶累及以单侧多发为主(63.63%)。骨显像特点包括:四肢骨和肋骨呈干状或条索状放射性浓集,但骨形不增大;颅骨和髂骨呈块状放射性浓集。结论OFD的99Tcm-MDP骨显像有特征性表现,如结合病史或X线检查有助于对该病的鉴别诊断。 Objective To analyze the characteristics of ^99Tc^m-MDP bone scintigraphy in patients with osteofibrous dysplasia (OFD) , and compare the images of primary bone malignancy and bone metastases with the findings of OFD. Methods Ninety-seven patients with OFD, surgically and pathologically comfirmed, were routinely performed scanning with ^99Tc^m-MDP bone scintigraphy. Abnormal findings from ^99Tc^m-MDP bone scintigraphy were reviewed and statistically analyzed. Results Eighty-six cases presented with monostotic OFD and mainly involved the extremities, especially the femoral bones (51.14%) , followed by tibia (27.90%). Eleven cases came up with poly OFD, frequently seen unilaterally(63.63% ). Typical imaging characteristics included linear accumulation of radioactivity at the foci despite normal bone scale in the ribs and extremities, while irregular bulky radioactivity was observed in cranium and pelvic bones. Conclusion Abnormal ^99Tc^m-MDP bone scintigraphic findings in patients with OFD may be helpful in the differential diagnosis of OFD.
