摘要 目的探讨无创心功能监测在冠状动脉旁路移植术后监护中的应用。方法选择2006年1~3月心外ICU收治的29例冠状动脉旁路移植术患者,应用胸腔生物阻抗法心脏血流动力仪和Swan-Ganz导管,同时测定患者的心排出量和肺动脉楔压,并对结果进行对比分析。结果两种方法测定的结果心排出量(CO)分别为(5.03±1.48)L/min和(5.25±1.55)L/min(P>0.05),r=0.82(P<0.001);肺动脉楔压(PCWP)的结果分别为(10.35±2.18)mmHg和(9.86±2.67)mmHg(P>0.05),r=0.71(P<0.05)。两种方法测定的结果具有良好的相关性。结论胸腔生物阻抗法无创心功能监测与Swan-Ganz导管测定结果是一致的,它不仅能准确地测定心排出量和肺动脉楔压,而且还具有无创、连续、简便和廉价的优点。 Objective To evaluate the noninvasive hemodynamic monitoring in patients undergoing coronary artery surgery. Methods Impedance cardiography technology and Swan-Ganz catheter were applied to measure the cardiac output and pulmonary capillary wedge pressure of 29 patients in Cardiac Surgical ICU. Results There was a significant correlation between impedance cardiography technology and Swan-Ganz catheter for cardiac output (5.03±1.48)L/min vs (5.25±1.55)L/min,P〉0.05,r=0.82,P〈0.001, and pulmonary capillary wedge pressure measurement (10.35±2.18)mm Hg vs (9.86±2.67)mm Hg,P〉0.05,r=0.71,P〈0.05). Conclusion There is close agreement between impedance cardiography technology and Swan-Ganz catheter in the measurement of cardiac output and pulmonary capillary wedge pressure in the patients. The impedance cardiography technology is a noninvasive, feasible, handy, continuous and cheap measurement of hemodynamic monitoring.
