摘要
目的:了解147株铜绿假单胞菌ampC基因分布和产酶状况对药敏的影响。方法:通过聚合酶链反应(PCR)法检测ampC基因分布情况,PCR产物克隆测序;通过酶提取物三维试验检测AmpC酶;纸片扩散法检测铜绿假单胞菌的药敏情况。结果:107株铜绿假单胞菌ampC基因阳性(占72.8%),PCR产物测序结果与Gene-Bank序列(AE 004827)同源性为100%。ampC基因阴性菌株依次对亚胺培南、头孢他啶、头孢吡肟及头孢哌酮/舒巴坦敏感,敏感率均≥90.0%;ampC基因阳性菌株对头孢他啶、头孢吡肟及头孢哌酮/舒巴坦敏感率明显降低(P<0.05);147株铜绿假单胞菌中88株产AmpC酶(占60.0%),不产AmpC酶菌株依次对亚胺培南、头孢哌酮/舒巴坦、头孢他啶、头孢吡肟敏感,产AmpC酶菌株仅对亚胺培南、头孢吡肟保持较高敏感率,而对头孢哌酮/舒巴坦、头孢他啶、头孢噻肟、氨曲南、头孢呋辛敏感率明显降低(P<0.05)。结论:铜绿假单胞菌广泛存在着ampC耐药基因,明确ampC基因分布和产酶状况有助于临床抗菌药物的选用。
Objective: To investigate the effects of ampC gene distribution and AmpC enzyme production in Pseudornonas aeruginosa on drug susceptibility. Methods: We detected ampC gene distribution by PCR, cloned and verified the amplified ampC genes by DNA sequencing, measured AmpC β-lactamase production by three-dimensional tests, and determined the susceptibility of Pseudornonas aeruginosato antimicrobial agents by disk diffusion tests. Results: The ampC gene was found to be positive in 97 out of 147 strains by PCR. The PCR product showed complete coincidence with the GenBank ampC sequence. The rate of susceptibility of ampC gene negative strains to imipenam, ceftazidime, cefepime and cefoperazone/sulbactam was ≥ 90.0%, while that of the positive strains to ceftazidime, cefepime and cefoperazone/sulbactam was significantly lower (P 〈 0.05 ). A total of 88 strains (60.0%) produced AmpC enzyme. The rate of susceptibility of AmpC enzyme negative strains to imipenam, cefoperazone/sulbactam, ceftazidime and cefepime was over 90%, while AmpC enzyme positive strains were only sensitive to imipenam and cefepime. Conclusion: Drug-resistant ampC genes extensively exist in Pseudomonas aeruginosa. The identification of ampC gene distribution and AmpC enzyme production in Pseudomonas aeruginosa is helpful to the selection of antimicrobial agents in the treatment of infection.
