股骨髁间粉碎性骨折预防高位髌的手术策略 被引量:2

摘要 目的探讨股骨髁间粉碎性骨折后髌滑道短缩而并发高位髌的防治策略和疗效。方法对26例股骨髁间粉碎性骨折实施植骨内固定术,同时以髂嵴—胫骨结节间距为受累股骨干长度参考,恢复股骨髁部解剖形态和髌股关节对应关系。结果本组术后膝关节X线侧位片提示Insall指数正常,随访23 d^31个月(平均13.8个月),按膝关节损伤功能评定标准,优良率达84.6%。结论股骨髁间粉碎性骨折可因骨缺损而出现髌滑道短缩,并导致高位髌。该类复杂骨损伤必须实施精确复位,在认真比对双侧髂嵴—胫骨结节间距基础上有效植骨和恢复髌滑道解剖形态是防止并发高位髌等髌股错格的有效措施。
