Effects of intrathecal MgSO_4 or/and morphine on incisional pain in the rat

摘要 目的观察在大鼠切口疼痛模型中鞘内注射(IT)硫酸镁(MgSO_4)或/和吗啡对大鼠切口疼痛的影响。方法雄性SD大鼠96只,随机均分为12组,每组8只,分别为假手术组、对照组、术前MgSO_4375μg治疗组和术后MgSO_4 375μg治疗组、术前MgSO_4 188μg治疗组和术后MgSO_4 188μg治疗组、术前吗啡5μg治疗组和术后吗啡5μg治疗组、术前吗啡2.5μg治疗组和术后吗啡2.5μg治疗组、术前MgSO_4188μg加吗啡2.5μg治疗组和术后MgSO_4 188μg加吗啡2.5μg治疗组。按Yaksh法施行鞘内置管。按Brennan法建立大鼠足底切口痛模型。分别于术前、术后2 h、3 h、6 h、12 h、1d、2 d、3 d、5 d以von Frey细丝法(机械性痛觉过敏)、热辐射法(热痛觉过敏)和累积疼痛评分法观察大鼠疼痛行为学变化。结果与假手术组和术前值比较,对照组、术前或术后MgSO_4 375μg治疗组、术前或术后MgSO_4 188μg治疗组、术前或术后吗啡2.5μg治疗组大鼠在术后2 h、3 h、6 h、12 h、1 d、2 d的累积疼痛评分均明显升高(P<0.05或P<0.01),von Frey纤毛刺激机械缩足反射阈值(MWT)均明显降低(P<0.01),热刺激缩足潜伏期(TWL)均明显缩短(P<0.05或P<0.01);与对照组比较,术前或术后吗啡5μg治疗组、术前或术后MgSO_4188μg加吗啡2.5μg治疗组大鼠的累积疼痛评分均明显降低(P<0.01),MWT均明显升高(P<0.01),TWL均明显延长(P<0.05或P<0.01)。结论在大鼠切口疼痛模型中,单次IT吗啡5μg或MgSO_4188μg加吗啡2.5μg能提供术后早期抗伤害作用。 Objective To observe the dynamic behavioral changes and effects of intrathecal MgSO4 or/and morphine on behavior in a rat model of incisional pain. Methods 96 male SD rats were divided randomly into twelve groups : sham group, control group, pre - incisional MgSO4 375 μg group, post - incisional MgSO4 375 μg group, pre - incisional MgSO4 188 μg group, post - incisional Mg- SO4 188 μg group, pre -incisional morphine 5 μg group, post - incisional morphine 5 μg group, pre - incisional morphine 2.5 μg group, post - incisional morphine 2.5 μg group, pre - incisional MgSO4 188 μg plus morphine 2.5 μg group and post - incisional MgSO4 188 μg plus morphine 2.5 μg group. PE - 10 catheter was inserted intrathecally to the lumbar region according to the method of Yaksh. A 1 cm long incision was made in the plantar region of left hindpaw according to the method of Brennan. Changes of pain behavior were assessed by withdrawal threshold to yon Frev filament stimulation intensity, response latency of the hindpaw to radiant thermal anda cumulative pain scores before incision as well as2 h , 3 h , 6 h , 12 h , 1 d , 2 d , 3 d , 5 d after incision. Results Compared with control group,there were no significant difference in withdrawal threshold to yon Frey filament stimulation intensity, response latency of the hindpaw to radiant thermal and a cumulative pain score in the pre - incisional and post - incisional MgSQ 375 tzg group, pre- incisional and post - incisional MgSO4 188 μg group, pre - incisional and post - incisional morphine 2.5 μg group, whereas in pre - incisional and post - incisional morphine 5 μg group, pre - incisional and post - incisional MgSO4 188 μg plus morphine 2.5 μg group the above three items were singnificantly changed. Conclusion In a rat model of postoperative pain, morphine 5 μg and MgSO4 188 μg plus morphine 2.5 μg given once either before or after incision may provide analgesia effects in early postoperative period.
