检测37例RA患者(15例伴贫血)血常规参数,疾病活动性指标(ESR及CRP)、铁代谢指标、叶酸、VitB_(12)、Epo及IL-1β等指标,并观察了rhIL—1β在体外对BFU—E和CFU—E的影响。结果显示RA贫血为轻—中度,贫血的发生可能与RA疾病活动有关;铁代谢指标观察10例符合ACD的特点,5例伴缺铁患者铁代谢指标的改变与单纯IDA相似,唯SF高于单纯IDA;RA患者FA、VitB_(12)的改变与贫血无关;血清Epo和IL—1β测定及IL—1β在体外对BFU—E和CFU—E的作用提示Epo对贫血的反应迟钝及IL—1β的升高可能参与了RA伴ACD的发生。
Blood picture, parameters of iron metabolism, folic acid(FA), vitamin B_(12) (Vit B_(12)), erythropoietin(Epo) and interleukin-1β (IL-1β) were measured in 37 patients with RA (including 15 patients accompanied anemia), and the effect of IL-1β on normal hemopoietic progenitors(BFU-E, CFU-E) in vitro was studied. The results were as follows: all anemia in RA were mild-or-moderate in extent. The degree of anemia was correlated with the activity of disease. In anemic patients, parameters of iron metabolism showed that 10 cases had feature of anemia of chronic disease (ACD) while 5 patients were similar to simple iron deficiency anemia (IDA), but serum ferritin was higher than that of simple IDA. Serum FA and VitB_(12) were normal mostly. Study on serum Epo and IL-1β and the effect of IL-1β on BFU-E and CFU-E in vitro indicated that a blunted Epo response to anemia and elavated IL-1β production might play some roles in pathrogenesis of ACD in RA.
