二维及三维磁共振胰胆管成像研究 被引量:1

摘要 目的探讨低场二维(two-dimensional,2D)和三维(threedimensional,3D)磁共振胰胆管水成像技术(magneticresonancecholangiopancreato-graphy,MRCP)对胆管系统显示的差别。方法采用日立AIRIS-Ⅱ0.3T永磁型磁共振(magneticresonance,MR),利用快速自旋回波序列(fastspinecho,FSE)及重T2WI成像技术(加大TR/TE,确保水为高信号),对40例受检者同时行2D及3DMRCP检查,获取的图像经计算机重建处理。两种方法对胆管显示利用配对资料χ2检验进行统计学分析。结果2D及3DMRCP均能很好显示胆管系统解剖结构,2DMRCP显示肝内3级胆管优于3DMRCP,两种成像技术差异无显著性意义(P>0.05)。结论低场强MR无论是2DMRCP还是3DMRCP,只要运用方法得当,都可以清晰显示正常胆管系统和胆管梗阻部位及其程度。但2DMRCP由于扫描时间短,几乎不受呼吸运动影响,所以较3DMRCP更适于胆管系统的检查。 Objective To explore difference of two-dimensional and three-dimensional magnetic resonance (2D and 3D MRCP ) and demonstrate the bile duct system Methods 2D and 3D MRCP was performed on 40 cases with fast spin echo (FSE) at 0.3 Tesla MR unit (AIRIS-Ⅱ ) and images were processed by reconstructing projection. Difference of two techniques showing biliary ducts was analyzed with paired Chi-square test. Results Anatomy of biliary ducts could be well demonstrated with either 2D or 3D MRCP. 2D MRCP could demonstrate biliary ducts (3 grade) much better than 3D MRCP, and the differences are not significant (P〉0.05). Conclusions When used properly, both 2DMRCP and 3DMRCP are very helpful to revealing the system of biliary ducts and to demonstrating the obstructive site and extent clearly. However, the scanning time of2D MRCP is short and has no effect of breath movement, so the 2D MRCP is more applicable than 3D MRCP for examining the bile duct system.
