摘要
目的探讨通过检测血清CagA抗体鉴定幽门螺杆菌高毒株感染的可行性。方法采集本地区临床门诊行胃镜检查的胃活检标本,作病理检查和幽门螺杆菌(Hp)培养,同时采集病人的血清。采用PCR法检测分离所得的HpcagA基因,用EIA法测定患者血清CagA抗体,分析cagA基因的检出与消化性疾病的关系,比较cagA基因与CagA抗体的检测结果,评价应用CagA抗体检测Hp高毒株感染的可行性。结果分离获得80株Hp,其中54株检出cagA基因,阳性率为67.50%。各病理类型胃病患者cagA基因阳性率分别为:浅表性胃炎77.42%,萎缩性胃炎62.86%,胃溃疡58.33%,胃癌50.00%,各组间无显著差异(P>0.05);EIA法检测CagA抗体的阳性率为55.00%,敏感度为74.07%,特异度为84.62%,约登指数(Youden's index)58.69%,调整一致性77.68%。结论检测血清CagA抗体用于诊断高毒力Hp感染存在局限性。
Objective To evaluate the feasibility of Mentification of high virulent strains of Helicobacter pylori by detection of serum CagA antibodies. Methods Gastric mueosa biopsy specimens by gastroseopy in outpatient departments in this area were collected for pathologic examination and culture of Helicobacter pylori , while the sera of same patients were collected simultaneousely. Hp eagA genes were isolated and tested by polymerase chain reaction and serum CagA antibodies were detected by enzyme immunoassay(EIA). The feasibility of identification of high virulent strains of Helicobacter pylori carried out by detection of serum CagA antibodies . The relationship between detection of eagA gene anddigestive diseases was analyzed and the results of detection of eagA genes and CagA antibodie were also compared. Results There 80 Hp strains were isolated from patients wherein 54 strains possessed eagA gene. Total positive rate ef cagAgene was 67.50%. The positive rates of eagA gene in various pathological types of gastropathy patients were as follows: superficial gastritis 77.42%, atropie gastritis 62.86%, gastric ulcer 58.33%, gastric carcinoma 50.00%, respectively ( P 〉 0.05). The total positive rate of CagA antibodies detected by EIA was 55.00%, while the sensitivity, the specificity, Youden' s index and the adjusted agreement were 74.07% , 84.62%, 58.69% and 77.68%, respectively. Conclusion Detection of serum CagA antibodies for diagnosis of high virulent Hp infection has its limitation.
