摘要
目的研究肢体水肿演变过程中不同时期的MR淋巴造影影像特征及其病理基础,探讨MR淋巴造影在肢体淋巴水肿方面的诊断价值。方法用改良的Danese手术方法在20只新西兰大白兔后肢一侧制作淋巴水肿模型,另一侧作为对照。在每只大白兔双侧后肢足背部趾蹼处注射0.2ml欧乃影,于淋巴水肿演变过程的不同时期进行三维MRI淋巴造影。成像条件为:1.5T超导型MR成像仪,使用标准矩形头线圈以获得最佳信噪比;梯度场20mT/m,切换率120mT/(m·ms);采用三维小角度激发快速梯度序列冠状位扫描,TR6.0ms,TE1.65ms,翻转角40°,视野(FOV)450~500mm,矩阵512×512,层厚1.0cm,三维块内共90层。结果MRI淋巴造影能准确地确定淋巴管阻塞的部位,反映淋巴管形态、功能的状况。肢体淋巴水肿的不同时期,由于其病理基础不同,产生不同的MR淋巴造影表现。结论间质MR淋巴造影可以在解剖背景下敏感而又可靠地显示各期肢体淋巴水肿情况。
Objective To study the imaging characteristics of interstitial magnetic resonance (MR) lymphography and its pathological basis in different stages of limb lymph edema and evaluate the diagnostic value of MRI in limb lymphedema. Methods The experimental model of lymphedema in one hind leg was established basing with modified Danese's method in 13 New Zealand White rabbits. Another hind leg of each rabbit was assigned as control limb. The 0.2 ml Gadodiamide were injected subcutaneously into the interdigitial skin of the dorsal aspect of each hind leg in 13 rabbits. The three-dimensional magnetic resonance lymphography (3D MRL) were performed at different stage of limb lymphedema. Histopathological examination of limb specimen was studied. Results Interstitial MR lymphography well displayed the obstructed position, configuration and status of function of the lymphatic vessel. The MR lymphographic findings varied from stage to stage due to different pathological changes. Conclusion The MR lymphography is sensitive and reliable for demonstration of limb lymphedema in different progressing stage under anatomical basis of lymphatic system.
