巩膜扣带术后视网膜中央动脉血流改变的影响因素 被引量:3

The relationship between retinal blood flow alterations associated with scleral buckling procedures and effect factors.

摘要 用多普勒超声对45例视网膜脱离扣带术后的检测显示,巩膜扣带术可引起视网膜中央动脉血流减慢,平均时间流速下降30%,阻力指数增高10%,较对照组差异有显著性。本文采用多因素逐步回归方法对15项可能影响血流速度的全身和局部因素进行分析探讨,结果表明:平均血流速度的影响因素依次为患者年龄、搏动指数、眼压、患者年龄、搏动指数,眼压与血流速度呈显著负相关;阻力指数的影响因素依次为搏动指数、收缩期峰值流速、病程、动脉硬化程度,病程,动脉硬化程度等与阻力指数呈显著正相关。提示:适宜掌握环扎带松紧度,特别是对伴有动脉硬化患者不宜过紧和术后保持眼压在正常较低水平,早期手术,合理使用末梢血管扩张剂等,有助于减轻血管阻力,提高眼血流灌注,减少眼缺血性并发症的发生。 Using Doppler imaging,we found that scleral buckling procedures may decrease central retinal artery blood flow.In this study,the relationship between retinal blood flow alterations and fifteen kind of effect factors were analysised with stepwise multivariate regression,The results showed that age of patients and intraocular pressure were correlated significantly negative to time average velocity of central retinal artery,while the process of history of retinal detachment and degree of atherosclerosis were positive to resistance index.The decrease blood flow postoperationlly was mainly caused by increase of vascular resistance.it is conclussion that administering some ending vascular dilatation drugs and regulating intraocular pressure at lower normally level were very useful to reduce the vascular resistance and increase ocular blood perfusion after scleral bucking.
