共同性外斜视内直肌胶原纤维超微结构观察 被引量:3

摘要 目的观察共同性外斜视患者内直肌胶原纤维的变化,探讨发生改变的意义。方法选择共同性外斜视患者12例为斜视组,角膜移植供体7例为对照组。取1mm3内直肌组织块,经固定脱水包埋,超薄切片染色后在JEM-1200EX型透射电镜下观察,对横断面进行图像分析,并做统计学处理。结果两组胶原纤维呈簇状穿插于内直肌纤维之间,排列不规则,形成三维结构,横切面呈规则的圆形或椭圆形。斜视组胶原纤维直径及横切平均面积与对照组相比差异均有统计学意义(P<0.05)。斜视组横切面积标准差也较对照组显著增大(P<0.05)。而胶原纤维分布密度斜视组与对照组差异无统计学意义(P>0.5)。结论共同性外斜视内直肌胶原纤维主要表现为直径增大,每个胶原纤维直径大小变异明显。 Objective Some researches reported the pathomorphological change of extraocular muscles and extracellular matrix in concomitant exotropia such as degeneration of muscle fibers,less cytochondriomes and structure abnormal in medial rectus. However,the changes of collagen fibril in these patients remained unclear. This paper was curried out for investigating the changes of collagen fibril in medial rectus muscles with concomitant exotropia. Methods Twelve 1 mm^3 specimen of medial rectus muscles from the patients with concomitant exotropic were collected during the surgery and 7 same size of specimen of medial rectus muscles from normal corpse eyes were obtained in control group. Sections of medial rectus muscles were observed under the transmission electron microscope. The crossed-section collagen fibrils was analyzed using Image Analyzer System. Results Cluster-like collagen fibrils arranged irregularly in different orientations,forming a three-dimensional network within the rectus muscle fibrils in the specimen from both groups. The fibrils showed the round and regular contour. In the exotropic group,the mean diameter and area of collagen fibrils (54. 23 ± 6. 865 nm,2 394. 43 ±624.44 nm^2 respectively) were significantly enhanced in comparison with the control group (46. 96 ± 4.97 nm, 1 764. 62 ± 368.77 nm^2 respectively) ( P = 0. 025 7,P = 0. 027 2 ). The thickness of collagen fibril was evidently increased in exotropic individual compared to the normal control( P 〈 0.05 ). However, the mean number of collagen fibrils per unit area was significantly higher in the exotropic group (180. 83 ± 58. 20/10^6 nm^2) than the control group (198.57 ± 63.27/10^6 nm^2 ) ( P = 0. 542 7 ). Conclusion The collagen fibrils in medial rectus muscles of patients with concomitant exotropia thick, and the heterogeneity of collagen fibril thickness in each individual is more obvious.
