同种肝细胞移植细胞排斥反应机理及肝再生增强因子的作用 被引量:7

The role of augmentor of liver regeneration in cytoimmunity of allogeneic hepatocyte transplantation

摘要 目的探讨同种异体肝细胞腹腔内移植治疗急性肝衰竭大鼠的细胞排斥反应机制及肝再生增强因子(ALR)的免疫作用。方法采用D-氨基半乳糖诱导大鼠急性肝衰竭(ALF),诱导后24h,随机分成5组。Ⅰ组:注射生理盐水;Ⅱ组:单纯移植肝细胞;Ⅲ组:肝细胞移植联合CsA;Ⅳ组:肝细胞移植联合ALR;Ⅴ组:注射ALR;肝细胞和药物均腹腔内注射。观察大鼠的存活率、组织学改变、移植肝细胞存活情况,检测大网膜CD4、CD8、CD68。结果Ⅳ组存活率显著高于Ⅰ组(66.7%vs0%,P=0.001),余组间无差异。Ⅳ组移植肝细胞存活时间更长。d1-2大网膜CD68阳性率Ⅱ组高于Ⅰ、Ⅳ组;CD4、CD8阳性率Ⅰ、Ⅱ组均高于Ⅳ、Ⅴ组,且Ⅱ组CD4阳性率高于Ⅲ组。d14大网膜CD4、CD8、CD68阳性率各组间无差异,但Ⅳ组CD4、CD8、CD68阳性率d1-2均低于d14的,Ⅱ组CD68阳性率d1-2高于d14的。结论同种肝细胞腹腔内移植联合ALR能提高ALF大鼠的存活率,同种肝细胞移植能引起CD4、CD8、CD68介导的细胞免疫,ALR能有效的抑制同种异体细胞免疫,促进移植肝细胞存活和增殖。 Objective To investigate the cellular rejection mechanisms of allogeneic hepatocyte transplantation intraperitoneally and the immunifaction of augmentor of liver regeneration (ALR) in acute liver failure (ALF) induced by D-galactosamine ( D-gal). Methods ALF rats were induced by D-gal. HcT was received 24 h after intoxication intraperitoneally, and the rats were divided into 5 groups randomly. Group I received normal saline (NS), group Ⅱ received hepatocytes, group Ⅲ received hepatocytes and CsA, group Ⅳ received hepato- cytes and ALR, and groupⅤ received ALR. The survival rate, the pathohistological examination of transplanted hepatocytes and liver, as well as the positive rates of CD4, CD8, and CD68 in greater omentum of all rats were observed. Results Survival rate of group Ⅳ was higher than that of group Ⅰ (66.7% vs 0%, P = 0. 001 ), while there was no significant difference among other groups was similar. Rate of positive d1-2 CD68 in group Ⅱ were higher those that of group Ⅰ and Ⅳ ; rate of positive and Ⅱ d1-2 CD4 and CD8 group Ⅰ were higher than those of group Ⅳ and Ⅴ ; rate of positive d1-2 CD4 in group Ⅱ was higher than that of group Ⅲ. The rate of positive d1-2 CD4, CD8, and CD68 was lower than that of d14 in group Ⅳ ; the rate of positive d1-2 CD68 was higher than that of d14 in group Ⅱ . Conclusion HcT plus ALR increase the survival rate of ALF rats intoxicated with D-gal. Allogeneic hepatocyte transplantation evokes CD4-, CD8-, and CD68-mediated cell immunity. ALR can suppress the cytoimmunity effectively, thus improve liver histology and promote proliferation of transplanted hepatocytes.
