成人阻塞性睡眠呼吸暂停综合征的局麻手术治疗

摘要 目的比较成人阻塞性睡眠呼吸暂停综合征(OSAS),全麻手术治疗即悬雍垂腭咽成形术(UPPP)与坐位局麻下行改良悬雍垂腭咽成形术优缺点。方法5年来,笔者对368例成年OSAS施行局麻坐位改良悬雍垂腭咽成形术,并对其进行术中术后并发症观察及术后效果回访。结果多导睡眠图(PSG)分析显示,术前、后血氧饱和度有明显不同(P<0.05),睡眠结构明显改变,呼吸暂停消失。无术中、术后并发症。结论坐位局麻下手术要优于传统全麻手术治疗,坐位局麻下悬雍腭成形术是可行的。  Objective To study the differences between conventional total-ansethetic uvulopalatophar-yngoplasty(UPPP) operation, i.e., the therapy of obstructive sleep apnea syndrome(OSAS), and the improved UPPP for OSAS under local-anesthetic, sitting and awaking state.Method The 368 cases had been subjected to improved UPPP in recent 5 years,and received investigation of post-operative effects and observation of complication during and after the operation.Results PSG showed that there were significant differences in blood oxygen saturation between pre- and post- operaton(P〈0.05),sleep structure was distinct changed; After operation apnea disappreared. No complication was found during and after operation.Conclusion Operation under local anesthesia and sitting state is better than the conventional one with general anesthesia for OSAS.
