摘要
目的观察缬沙坦对高血压合并持续性心房颤动(房颤)患者在降压同时对其心率(HR)、血压(BP)、左心房结构及左心室功能的影响。方法对2004年1月至2005年6月哈尔滨医科大学第一临床医学院收治的高血压合并持续性房颤患者106例,在控制心室率、预防血栓治疗的基础上应用缬沙坦80mg/d,对照组用依那普利10mg/d。3个月、6个月后对照观察其对心率、血压、左心房结构及左心室功能的影响。缬沙坦组54例,依那普利组52例。结果分别观察3个月、6个月后两组血压、心率均控制理想,两组间差异无显著性意义(P>0.05)。左心房直径缬沙坦组治疗前平均(50±7)mm,3个月后为(46±4)mm,6个月后为(40±3)mm,与治疗前相比差异有显著性意义。依那普利组3个月、6个月后左心房也有缩小,但差异不显著。治疗组治疗前NY分级Ⅱ、Ⅲ级占64%,治疗后3个月、6个月分别下降至33%、15%。而依那普利组也有明显下降,3个月、6个月分别下降至45%、25%,下降程度不如缬沙坦组明显。结论长期应用缬沙坦(3个月以上)可以显著改善左心房结构、左心室功能和持续性房颤患者的长期预后。
Objective To evaluate the influence of Valsartan on HR,BP,LA structure and LV function of the patients with persistene atrial fibrillation and hypertentionMethods A total of 106 patients（from Jan,2004 to June,2005 in our hospital）with persistene atrial fibrillation and hypertention had been treated by heart rate control and thrombosis therapy. The therapy group was treated with Valsartan 80 mg/d,and the contrast group was treated with enalapril 10mg/day. Then,we observed the effect on HR,BP,LA structure and LV function. Results After three months and six months,the two group（54 patients with Valsartan and 52 patients with enalapril） both had good HR control and no there was significant difference. In the group with Valsartan,the diameter of LA was（50±7）mm before therapy,（46 ±4）mm after 3 months and（40 ±3）mm after 6 months. In the the group with enalapril,the diameter of LA was diminished too,but not obviously. The study suggested that although the group with enalapril had smaller LA, the difference was not significant. We estimated the LV function by clinical signs. In the therapy group ,64% patients was with NY ⅡorⅢ before therapy,but 33% after 3 months,and 15% after 6 months. In the group with enalapril ,the patients were reduced to 45% after 3 months,and 25% after 6 months,which was not reduced obviously. Conclusion This study presented by long-term observation （ 〉 3 month）, that Valsartan can improve LAreconstruction significantly, and improve LV function,as well as the long-term prognosis of persistent AF.
