摘要
目的探讨经阴道超声检查(TVS)结合多变量Logistic回归分析法对早期异位妊娠(EP)的诊断价值。方法运用TVS检测和观察97例确诊的早期EP患者的子宫内膜厚度、对称性、回声、形态等二维声像图特征,进行单、多因素Logistic回归分析和ROC曲线分析,找出有意义的变量和建立预测方程。结果子宫内膜三线状改变及内膜厚度≤9mm是有意义的超声诊断变量,OR值分别为299.59和34.139,多变量Logistic预测方程的建立比单独应用单一变量拥有更大的曲线下面积,能够更好地鉴别诊断EP。结论TVS对EP的早期诊断敏感性较高。TVS结合多变量Logistic回归分析能克服用单一变量指标作为诊断和鉴别诊断标准时敏感性较低的问题,能更准确地预测EP。
Objective To explore the value of the combination of transvaginal ultrasound(TVS) and multivariate Logistic regression in the diagnosis of early ectopic pregnancy(EP).Methods The two-dimensional endometrial characteristics(thickness,symmetric,resonance,morphology) of 97 definitely diagnosed early EP patients were detected and observed by TVS.After the univariate and multivariate Logistic regressive analysis and ROC analysis were performed,the significative variates were found out and the predictive equation established.Results The tri-linear changes of endometrium and whether the thickness of endometrium was less than 9 mm were the significative variates of ultrasonic diagnosis.The OR values were 299.591 and 34.139 respectively.The area under curve of multivariate Logistic predictive equation was larger than that of single variants,so it was a better way to diagnose EP differentially.Conclusion TVS had high sensitivity in the diagnosis of early EP.The combination of TVS and multivariate Logistic regression analysis could predict EP more exactly.It could solve the low sensitivity of single variable as the only diagnostic and differential diagnosis criteria for the early EP and improve the reliability of prediction.
