摘要
目的:探讨血清脑钠素(BNP)水平在评价心力衰竭(HF)严重程度中的应用价值及观察不同病因HF患者血清BNP的水平变化。方法:选取明确有HF的NYHAⅡ级、Ⅲ级、Ⅳ级的病人89例,23例心功能正常者为对照。采用全自动免疫分析仪(罗氏2010型)电化学发光法(ECLA)测定各组HF患者血清BNP水平。结果:1HF病人的血清BNP为4745.49±3698.70ng/ml,高于心功能正常者66.18±44.30ng/ml,两组相比差异显著(P<0.05),且随着心功能分级的增加BNP也相应增加,Ⅳ级(10251±9326.8ng/ml)Ⅲ>级(1239±1063.8ng/ml)Ⅱ>级(167.4±143.4ng/ml),均有显著差异(P<0.05)。2不同病因(高血压、冠心病、风心病)导致的HF血清中BNP水平(Ⅲ、Ⅳ级)的差异P>0.05,但级有显著差异(P<0.05)。结论:1HF患者血清BNP水平显著增高,且随着NYHA分级增加而增加,可作为评价HF严重程度的客观指标。2不同病因HF患者(Ⅲ级、Ⅳ级)血清BNP水平与疾病无明显差异,但Ⅱ级HF患者的血清BNP水平与疾病有差异。
Objective: To examine the value of Brain Natriuretic peptide (BNP) in estimating the degree of seriousness of heart failure (HF) and study the mechanism of the elevation of BNP in HF caused by various heart diseases. Methods: 89 cases of HF were divided into four groups by classification of New York Heart Association (NYHA) in reference to 23 healthy individuals as control group. Samples of serum BNP concentration were analyzed by electrochemical luminescence assay (ECLA), followed by comparisons of BNP levels among all groups. Result :①Average serum BNP concentration was 4745.49± 3698.70 ng/ml,and it was higher in HF group than that in control group(66.18±44.30ng/ml,P〈0. 05). Moreover, BNP level increases with decreasing grades.. Grade 4(10251±9326.8 ng/ml)〉 Grade 3 (1239 ±1063.8 ng/ml)〉 Grade 2(167.4±143.4 ng/ml,P 〈0. 05), all statistically meaningful. ②There was no related to positive correlation between the levels of serum BNP and HF in Grade 3 and Grade 4, but in Grade 2 the levels of serum BNP were hypertension, coronary artery disease or rheumatic heart disease. Conclusion:① Serum BNP levels of HF patients are significantly higher in comparison to normal individuals. Serum BNP levels increases in accordance to NYHA grades. Therefore, Serum BNP level can be applied as an objective indicator of the severity of HF diseases. ② For patient categorized into Grade 3 and 4 in NYHA, causes of HF diseases are not statistically related to their serum BNP levels. However, for Grade 2 patients, their BNP levels vary among different HF causes.
