围体外循环心脏手术期预防性应用抗菌药物调查分析 被引量:4

摘要 目的:调查某三甲医院围体外循环心脏手术期预防性应用抗菌药物的合理性。方法:回顾性调查2006年1月~2006年12月在该院行体外循环心脏手术病历202份。统计分析围手术期使用抗菌药物种类、给药途径、用法用量、疗程、用药频度和药物利用指数等。结果:手术切口全部为Ⅰ期愈合。手术前、后均预防性应用抗菌药物预防感染,抗菌药物应用种类以第二代头孢菌素和青霉素类药物为主,190例患者(94.06%)为单用一种抗菌药物,92.08%的患者为手术前24h预防性应用抗菌药物,术后应用抗菌药物疗程1d~10d不等,大部分病例(79.7%)于术后72h内停用抗菌药物。结论:该院围体外循环心脏手术期预防性应用抗菌药物基本合理,但尚存在个别不当之处,如选用抗菌药物针对性不强、缺乏病原学依据和用药疗程偏长的问题,有待进一步规范。 Objective:To explore the rationalization of antibacterial agent prophylaxis during the perioperation period of heart operation by extracorporeal circulation.Methods:202 cases receiving heart operation by extracorporeal cireulation from January 2006 to December 2006 were retrospectively studied.The investigative contents included operation type, the category of antibacterial agent, administration way, dose and dosage, course of treatment, DDDs, DUI, et al.Results:The operation incisions of 202 cases all healed in first stage and all cases were given antibacterial agent to prevent infection during the perioperation period including 190 cases (94.06%) given one kind of antibacterial agent. The antibacterial agents used in these cases mainly were the second generation of cephalosporin and broad-spectrum penicilins.Most cases (92.08%) received antibacterial agent 24 hours before operation and treated in the 72 hours after operation (79.7%).Conclusion:Antibacterial agent prophylaxis used in these cases was mainly rationable, but there still was some problems such as choosing inpropriate drugs which were poor antibacterial activity to common pathogens and long treatment.So we should take some measures to further standardize the prophylactic use of antibacterial agent during the perioperation period of heart operation by extracorporeal circulation.
