摘要 目的:对甲真菌病的临床特征进行深入研究,以期发现作为初步诊断的临床线索。方法:收集甲真菌病及非甲真菌病病例,详细记录患者的一般情况、病史及病甲特征,并进行统计分析。结果:(1)共收集124例甲真菌病和29例其它甲病病例,真菌培养阳性者84例。(2)有6项病史(手足多汗史,浅部真菌病史,密切接触者中有浅部真菌病,掌跖脱屑史,趾指间浸渍糜烂史,掌跖/趾指腹水疱史)和2项临床体征(掌跖脱屑,单侧指甲受累)高度提示甲真菌病。结论:甲真菌病有某些特征可作为临床诊断的线索。 Objective: To estabhsh a clinical diagnostic clue for the diagnosis of onychomycosis, based on clinical characteristics of the patients with onchomycosis. Methods: The general condition, clinical history and clinical characteristics of patients with nail disorders were collected and analyzed. Mycological culture was done for all specimens taken from the patients. Results: ( 1 ) A total of 124 cases of onychomycosis and 29 cases of other nail diseases were collected. Seventy - five cases were positive on culture for Trichophyton rubnm, and 6 cases for Cand/da spp, 1 case each for Fusarium oxysporum, Malassezia pachydermatis and coinfection of Candida albicans and Trichophyton rubrum, respectively. (2) Eight parameters were found to be significantly related to positive mycology results, including 6 clinical indicators (hyperhidrosis on the palm and sole, superficial mycosis of skin, close contact with people suffering from superficial mycosis, scaly skin on sole/palm, interdigital maceration, history of vesicles on sole/ palms/ventral aspects of toes or fingers) and 2 chnical findings (scaling on sole/palm, nails on unilateral nail involvement) .Conclusion: Some chnical features can be used as clue in the diagnosis of onychomycosis.
