摘要
目的:探讨胆管细胞癌中脆性组氨酸三联体(fragile histidine triad,FHIT)和周期素D1(Cyclin D1)基因蛋白表达状况及其与临床病理的可能关系。方法:采用兔抗人FHIT和鼠抗人Cyclin D1蛋白抗体和枸橼酸-微波-非生物素二步法免疫组化技术,检测53例胆管细胞癌、30例癌旁组织和20例正常胆管组织标本中FHIT和Cyclin D1表达状况,并分析其与患者年龄、性别、肿瘤部位、组织学分级以及临床分期的关系。结果:胆管细胞癌FHIT表达较癌旁组织和正常胆管组织明显降低,P<0.001,而Cyclin D1表达却较癌旁组织和正常胆管组织明显升高,P<0.001。FHIT蛋白表达与年龄、性别及肿瘤部位无关,P值分别为0.776、0.246和0.347,但与癌组织学分级及临床分期相关,P值分别为0.007和0.004。Cyclin D1蛋白表达与年龄、性别及肿瘤部位无关,P值分别为0.965、0.751和0.948,但与癌组织学分级及临床分期相关,P值分别为0.043和0.047。胆管细胞癌FHIT表达与CyclinD1表达呈负相关,P<0.001。结论:FHIT表达缺失和Cyclin D1高表达可能提示其抑癌功能丧失,对胆管细胞癌的发生、演化和进展具有重要作用。FHIT表达缺失及Cyclin D1高表达可能成为辅助胆管细胞癌诊断及预后的一个新标志。
OBJECTIVE:To investigate the expression of fragile histidine triad (FHIT) gene protein and Cyclin D1, and the possible relationship between FHIT or Cyclin D1 expression and clinicopathological indices in cholangiocellular carcinoma. METHODS: FHIT and Cyclin D1 protein expressions were detected in formalinfixed, paraffin-embedded tissues of 53 cholangiocellular carcinomas,30 adjacent non-neoplasms and 20 normal bile ducts by the two-step non-biotin immunohistochemical method and their relationships to patient's age, sex, site, histological grade,and clinical stage were analysed. RESULTS: FHIT expression in cholangiocellular carcinoma was significantly lower than that in adjacent non-neoplastic or normal bile duct tissues (P〈0. 001). Carcinomas with negative expression of FHIT were significantly corresponding to the histological grade(P=0.007)and clinical stage (P=0.004), but not to the age(P=0. 776), sex(P=0.246 ) andsite (P=0.347), respectively. On the other hand, Cyclin D1 expression in cholangiocellular carcinoma was significantly higher than that in adjacent non- neoplastic or normal bile duct tissues (P〈0. 001). Cancers with overexpression of Cyclin D1 were statistically correlated with the histological grade(P = 0. 043) and clinical stage ( P = 0. 047), but not with the age(P=0.965), sex (P=0.751) and site (P= 0. 948), respectively. The expression of FHIT was significantly invertedly related to the expression of Cyclin D1 (P〈0. 001). CONCLUSION: The loss of expression of FHIT and overexpression of Cyclin D1 may be associated with the carcinogenesis of cholangial epithelia, and the differentiation, development and the prognosis of cholangiocarcinoma, thus may become two new adjunct diagnostic and prognostic markers in cholangiocelluar carcinoma.
