摘要
目的 探讨腔内激光联合多种手术治疗下肢浅静脉曲张的疗效.方法 280例患者（297条患肢）根据静脉疾病的临床-病因-解剖-病理生理诊断系统中临床C分级,采用激光和/或联合多种手术治疗.全组均行腔内激光及大隐静脉高位结扎术;对C3～4另联合小切口行曲张静脉和交通支切除;对C5～6再增加股浅静脉瓣膜环缩术;1例Cockett综合征患者放置左髂静脉支架后,根据C分级完成手术.结果 243例随访1～18个月,平均6.3个月,随访患者均疗效满意.结论 静脉腔内激光治疗适用于大隐静脉及其属支功能不全患者;对于C4～6级患者,联合应用曲张静脉团点状切除、交通支离断和深静脉修复手术,临床效果更显著.
Objective To evaluate the results of endovenous laser treatment with concomitant surgical operation of varicosities in lower extremities according to clinical-etiology-anatomic-pathophysiologic （CEAP） classification system. Methods 297 lower limbs in 280 patients underwent laser therapy with concomitant surgical intervention according to CEAP classification. Endovenous laser therapy combined with high ligation of great saphenous vein was for all patients; additional varicosities and perforators ablation through several tiny incisions were for C3-4 patients; added external banding valvuloplasty of superficial femoral vein was for C5-6 patients; one Cockett＇s syndrome was treated according to C classification with stent placement in left iliac vein. Results 243 patients were followed up for 1-18 months （mean 6.3 months） with satisfactory results, and a few recurrences were observed. Conclusion Endovenous laser therapy is a new process for superficial veins of lower limb. It can be used for progressing varicosities in combination with surgery as well.
