摘要
目的探讨血清铁和红细胞平均体积(MCV)在3月~2岁婴幼儿地中海贫血(地贫)筛查中的应用价值。方法选择一年间到我院进行地贫检查婴幼儿374例资料,进行MCV、红细胞脆性、血红蛋白(Hb)电泳、血清铁的检测,以基因诊断为确诊试验对这些项目的检测结果进行回顾性分析。结果MCV、红细胞脆性、Hb电泳、MCV红细胞脆性Hb电泳三顶联合检测、血清铁MCV联合检测的灵敏度和准确度分别为:97.4%、95.7%、74.8%、98.3%、94.7%和67.4%、70.8%、78.6%、66.6%、97.1%。血清铁MCV联合检测结果的准确度最高,经U检验,与其它项目对比均有极显著性差别。结论血清铁MCV联合检测是3月~2岁婴幼儿地中海贫血筛查的更理想的试验方法。
Objective: To study the value of the combined test of serum iron and mean corpuscular volume (MCV) for the diagnosed of thalassemia in the infants and young children aged 3 month - 2 year old. Methods: Selecting 374 infants and young children datas from my hospital for this one year, review analyzed the results of the MCV, Red Cell Osmotic fragility ( RCOF), Hemoglobin (Hb) eleetrophoresis and serum iron. Results: The sensitivity and accuracy of MCV, RCOF, Hb electrophoresis, combination of three tests, combination of serum iron with MCV wore as follow: 97.4%, 95.70%, 74. 8%, 98. 3%, 94. 70% and 67. 4%, 70. 8%, 78.6%, 66.6%, 97. 1%. There were significant differeneees of accuracy between combination of serum miron with MCV and other tests. Conclusion : The combination of serum iron with MCV were the better test for the diagnosis for thalassemia in the infants and young children aged 3 month ~ 2 year old.
