哌甲酯对慢性阻塞性肺病患者吸气肌功能的影响及作用机理探讨 被引量:2

摘要 为探讨哌甲酯对吸气肌功能的影响,作者采用随机对照方法观察了70例慢性阻塞性肺病(COPD)患者静脉注射治疗剂量哌甲酯等药物前后最大吸气口腔压(MIP)、最大吸气中段流量(MMIF)、用力深吸气量(FIC)、最大吸气功(Wimax)、口腔阻断压(P0.1)及每分钟通气量(VE)等呼吸力学参数的变化,并与氨茶碱、可拉明比较。结果显示:用哌甲酯和氨茶碱后MIP、MMIF、FIC、Wimax、P0.1及VE均明显增加;用可拉明后P0.1增加更明显,但是反映吸气肌力的MIP、MMIF、FIC、Wimax参数却无明显变化。表明哌甲酯同氨茶碱一样,能明显增强COPD患者的吸气肌功能。 To have a better understanding of the effect of methylphenidatum on inspiratory muscles function, we studied the respiratory force parameters of 70 patients with chronic obstructive pulmonary disease by intravenous infusion methylphenidatum in a randomized controlled clinical trial. The indices of respiratory force parameter included maximal inspiratory mouth pressure (MIP), maximal midinspiratory flow (MMIF), forced inspiratory capacity (FIC), maximal works of inspiration (Wi max ) and airway occlusion pressure (P 0.1 ), etc. Aminophylline and Nikethamidi were chosen as controls. The results showed that MIP, MMIF, FIC, Wi max , P 0.1 and minute ventilation (V E) were significantly increased after administration of methylphenidatum and aminophylline. There were no significent differences in MIP, MMIF, FIC and Wi max after administration of Nikethamidi, but P 0.1 was significantly increased and the increase was higher than that after administration of methylphenidatum and aminophylline groups. We conclude that methylphenidatum can significantly improve the function of inspiratory muscles as aminophylline can do.
