摘要
目的探讨窦性心率震荡对急性脑梗死患者死亡的预测价值。方法回顾性分析急性脑梗死患者98例与对照组30例、急性脑梗死存活组75例与死亡组23例的24h动态心电图,比较各组间HRT的TO和TS。结果(1)与对照组TO、TS(-6.1±3.7%、13.1±5.6ms/R-R间期)比较,急性脑梗死组TO高(-3.2%±6.3%)、TS低(6.1±6.0ms/R-R间期),差异均有显著性意义(P<0.05、<0.01)。(2)与急性脑梗死存活组TO、TS(-5.2±5.7%、7.0±2.1ms/R-R间期)比较,死亡组TO值高(-2.2±4.3%)、TS低(5.4±4.0ms/R-R间期),差异均有显著性意义(P<0.05)。结论HRT与急性脑梗死患者的死亡及预后有关,可作为急性脑梗死患者预后的预测因子。
Objective To analyze clinical value of heart rate turbulence （HRT） in acute cerebral infarction （ACI）. Methods HRT onset （TO） and HRT slope（TS） were calculated based on Holter recorded in 75 survivors ofACI, 23 cases died of ACI and 30 normal subjects. Results （1） TO was higher and TS was lower in ACI patients than in normal subjects （TO -3.2±6.3% vs. -6.1±3.7% ,P〈 0.05,TS 6.1± 6.0 ms vs. 13.1±5.6ms / RRinterval, P〈0.01）. （2） TO was higher and TS was lower in cases died of ACI than in survivors of ACI （TO -2.2±4.3% vs. -5.2±5.7%, P〈 0.05,TS 5.4 ±4.0 ms vs. 7.0±2.1ms / RRinterval, P〈0.05）. Conclusion HRT is relative to death and prognosis of patients with ACI.
