摘要
目的:分析LASIK治疗近视眼的并发症,探讨其原因与预防。方法:使用Allegretto准分子激光机和Moria-M2微型角膜板层刀,连续对1100例近视者(2151眼)实施常规LASIK手术矫正。术后随访3～30mo。结果:在随访期内,97%的眼达到或超过术前最佳矫正视力,无角膜感染、无弥漫性层间角膜炎和激素性高眼压等严重并发症。术中并发症有:游离瓣4眼(0.19%),角膜瓣破损4眼(0.19%),小角膜瓣4眼(0.19%),不完全角膜瓣5眼(0.23%),角膜上皮损伤15眼(0.70%),角膜瓣边缘出血67眼(3.11%),球结膜出血214眼(9.95%)。术后并发症有:干眼症状710眼(33.01%),夜间光晕802眼(37.28%),视力波动788眼(36.63%),点状角膜表层细胞脱落410眼(19.06%),角膜高阶波前像差增大805眼(37.43%),过矫20眼(0.93%),欠矫43眼(2.00%),屈光回退48眼(2.23%),角膜瓣层间异物22眼(1.02%),丝状角膜病变2眼(0.09%),角膜瓣下上皮内生2眼(0.09%),单疱病毒角膜炎1眼(0.05%)。结论:LASIK手术要求术者经验丰富,设备性能精良。尽管如此手术并发症在所难免。
AIM： To analyze the complications of laser in situ keratomileusis （LASIK） for myopia and to discuss the masons and prevention of these complications.
METHODS： LASIK with Allegretto excimer laser and Moria-M2 microkeratome were performed on 1 100 patients with myopia （2 151 eyes）. The follow-up ranged from 3 to 30 months.
RESULTS： During the follow-up period, the uncorrected visual acuity （UCVA） in 97% of eyes reached or exceeded the preoperative best spectacle-corrected visual acuity （BSCVA） with no infection of cornea, diffuse lamellar keratitis or corticosteroid induced ocular hypertension occurred. Intraoperative complications included free flap in 4 eyes （0.19%）, damaged flap in 4 eyes （0.19%）, small flap in 4 eyes （0.19%）, incomplete flap in 5 eyes （0.23%）, corneal epithelial defect in 15 eyes （0.70%）, corneal limbal pannus bleeding in 67 eyes （3.11%） and conjunctival hemorrhage in 214 eyes （9.95%）. Postoperative complications were mild, including dry-eye symptoms in 710 eyes （33.01%）, night glare and halo in 802 eyes （37.28%）, fluctuate visual acuity in 788 eyes （36.63%）, punctual epithelial defect in 410 eyes （19.06%）, corneal wave aberration increase in 805 eyes （37.43%）, overcorrection in 20 eyes （0.93%）, undercorrection in 43 eyes （2.00%）, refractive regression in 48 eyes （2.23%）, interface foreign bodies in 22 eyes （1.02%）, filamentary keratitis in 2 eyes （0.09%）, epithelial ingrowth in 2 eyes （0.09%） and herpes simplex keratitis in 1 eye （0.05%）.
CONCLUSION： LASIK operation requires rich experience of surgeon and superior equipment. Even so the complications are unavoidable.
