男性糖尿病患者踝臂指数与全因和心血管病死亡率关系的前瞻性研究 被引量:11

Prospective investigation on association of ankle brachial index with all-cause and cardiovascular disease mortality in male patients with type 2 diabetes

摘要 目的研究男性糖尿病患者外周动脉疾病(PDA)的危险因素及踝臂指数(ABI)与全因和心血管病(CVD)死亡率的关系。方法研究对象来自2004年7月至2005年1月北京及上海地区8所医院的有完整ABI基线资料的男性糖尿病患者815例,于2005年11月至2006年1月对其进行随访调查。结果815例男性糖尿病患者中,ABI降低的PAD组250例(30.67%),ABI正常的非PAD组565例(69.33%),年龄和吸烟史是PAD的独立危险因素,在13.6个月的随访中,有70例死亡,其中27例为CVD死亡。经Cox回归分析,PAD患者发生全因及CVD死亡的相对危险度分别为1.744(1.056~2.877)和3.677(1.588~8.509)。PAD组的生存率显著低于非PAD组。随ABI水平的降低,发生死亡及CVD死亡的危险增加。结论低ABI是男性糖尿病患者死亡和CVD死亡的独立危险因素,在男性糖尿病人群中运用无创性ABI测定对全因死亡及CVD死亡具有预测价值。 Objective To evaluate the risk factors for peripheral arterial disease (PAD) and the association of the ankle brachial index (ABI) with all-cause and cardiovascular disease (CVD) mortality in Chinese male patients with type 2 diabetes. Methods The ABI cohort study was initiated in July 2004 to investigate risk factors of PAD and the relationship between ABI and all-cause and CVD mortality in 815 type 2 diabetic Chinese male patients of Beijing and Shanghai areas, ABI was ascertained at baseline by measuring the systolic blood pressures on bilateral braehial and tibial arteries. Mortality surveillance was completed from November 2005 to January 2006. Results Among 815 male participants with type 2 diabetes at baseline, 250 (30.67%) were in low ABI group and 565 (69.33%) were in normal ABI group. Aging and smoking were associated with low ABI in male patients with type 2 diabetes. During the 13.6-month follow-up, there were 70 deaths, of which 27 were attributable to CVD. Low ABI was associated with adjusted all-cause and CVD mortality risk of 1. 744( 1. 056-2. 877 ) and 3. 677 ( 1. 588-8. 509 ) respectively in Cox regression models. The rate of survival in the low ABI group was significantly lower than that in the normal ABI group. The risk of all-cause and CVD mortality was increased with the lowered ABI. Conclusion Low ABI was independently associated with the high risks of all-cause and CVD mortalities in Chinese male patients with type 2 diabetes. The utility of non-invasive ABI as a tool for predicting mortality in diabetic patients should be popularized.
