头部降温加尼莫地平对犬脑复苏的影响 被引量:2

EFFECT OF BRAIN COOLING AND NIMODIPINE ON RESUSCITATION OF EXPERIMENTAL CEREBRAL ISCHEMIA IN DOGS

摘要 犬16条分为3组.A组为无缺血实验对照组,犬4条,仅做分离颈部血管手术.B组为缺血实验对照组,犬6条.AB两组其后仅用一般处理.C组犬6条,脑缺血后再灌流时即静注尼莫地平10μg/Kg,另外10μg/Kg于30分钟内滴注完,同时将鼻咽温度降至28士1℃,直肠温度33士1℃,维持8小时.全麻用硫喷妥钠20mg/Kg和琥珀酰胆碱0.1g静注.气管插管,机械控制呼吸.双侧颈动脉和椎动脉夹闭加全身降压造成15分钟完全性脑缺血.复灌流后观察发现:C组第4小时起其神经缺陷积分即显著性地低于B组,14小时后全部恢复到零.24小时后A组和C组的动物均全部存活,存活率显著高于B组(全部死亡).结果表明,脑缺血后头部重点降温合并静脉用尼莫地平治疗在15分钟脑缺血模型中显示具有促进神经功能恢复,提高存活率等作用. Mongrel dogs weighing 10-17 kg were anesthetized with thiopentone 20mg/kg by intravenous injection and succinycholine 100 mg infusion. Animals were immediately intubated and placed on a ventilator. Complete cerebral ischemia was produced by ligation of bilateral common carodic and vertebral arteries under controlled hypoten-tion. After 15 minutes of complete cerebral ischemia, the brain was cooled with iced water until nasal temperature reached 28 C ± 1 C and rectal temperature 33 C ± 1 C . Then an infusion of nimodipine 20μg/kg intravenously over 30 minutes in the treated animals (group C [n = 6]) The control animals, were keeped in normal temperature and normal saline intravenously (group B[n=6]). Selective brain cooling and nimodipine intravenously was associated with significantly improved neurologic functions and with 100% survived, whereas control dogs showed marked neurologic dysfunction and with 100% mortality. We conclude that brain cooling and nimodipine intravenouly started immediately after 15 minutes of complete cerebral ischemia had a beneficial effect on the recovery of neurologic functions in experimental dogs.
