摘要
目的评价改良自制聚丙烯网片及穿刺锥经闭孔尿道中段悬吊治疗女性压力性尿失禁(stress urinary incontinence,SUI)的安全性和治疗效果。方法选择2005年1月至2005年12月确诊为单纯性SUI患者86例,分为2组,改良组40例,采用自制聚丙烯网片及穿刺锥行闭孔尿道中段悬吊术(改良TVT-O术);对照组46例采用TVT-O术。对2组患者术中情况及术后疗效进行定期随访,对比分析。结果2组患者均在静脉麻醉下完成手术。改良组平均手术时间(27±6)min,对照组(45±5)min;改良组平均术中出血(50±5)ml,对照组(80±5)ml;2组患者术后6 h去除导尿管后均能自行排尿,2组各有1例剩余尿>100 ml,经留置导尿48h后好转;2组患者均无膀胱损伤等相关并发症发生;改良组平均住院(5.0±0.5)d,平均住院费(2462±160)元,对照组平均住院(7.5±0.5)d,平均住院费(8956±230)元;改良组在手术时间、出血量、住院时间、治疗费用均明显少于对照组,差异有统计学意义(P<0.01)。改良组手术有效率90%(36/40),好转率10%(4/40);对照组手术有效率87%(40/46),好转率13%(6/46),2组均无无效或加重患者。2组患者手术疗效对比,差异无统计学意义(P>0.05)。结论改良自制聚丙烯网片及穿刺锥TVT-O术方法简单、操作安全、悬带调节方便、术后恢复时间短,效果确切,且价格低廉,易为患者接受。
To evaluate the efficacy and security of applying polypropylene mesh and screw used for female stress urinary incontinence of tension-free vaginal tape Obturator (TVT-O). Methods From January 2005 to December 2005, a total of 86 women diagnosed stress urinary incontinence were treated with the pubovaginal sling procedure using polypropylene mesh and screw (n=40) or traditional TVT-O (n= 46). Surgical results and urodynamic changes after the pubovaginal sling procedure were compared at follow up. Results All the patients were operated under local anesthe sia. Average operating time was (27±6)min in the self-made mesh group and (45±5)min in the traditional mesh group, mean blood loss was (50±5)ml and (80± 5)ml, respectively. All the patients were able to micturate spontaneously 6 h after operation; including 1 case with volume of residual u rine was more than 100 ml in each group. In the remaining cases, an indwelling catheter had to be kept for 48 h. The self-made mesh group had a shorter duration of hospitalization with (5.0±0.5)d, as the normal mesh group with (7.5±0. 5)d, and mean costing in (2462±160)RMB vs (8956±230) RMB. There was no bladder damage during the operation. Compared with TVT O group, there was shorter average operating time and duration of hospitalization, less mean blood loss and costing(P〈 0.01).
