2型糖尿病合并下肢动脉血管病变患者血浆脂联素的变化研究

摘要 【目的】探讨2型糖尿病患者及其合并下肢动脉血管病变患者血浆脂联素水平的变化。【方法】选取2型糖尿病患者58例(A组),2型糖尿病合并下肢动脉血管病变患者72例(B组),健康对照者组48例(C组)作为研究对象,进行彩色多普勒超声诊断仪(CDFI)下肢动脉检查,观察血管内径、流速、流量,并测定血浆脂联素水平、空腹血糖、胰岛素、血脂等生化指标,比较血浆脂联素水平的改变与下肢动脉血管病变的相关性。【结果】A组血浆脂联素水平低于C组(P<0.01),B组血浆脂联素低于A组(P<0.01);B组流速、血管内径、血流量较A组减少(P<0.01),A组较C组减少(P<0.05);血浆脂联素水平与下肢血管病变、FBG、BMI呈负相关,与HDL呈正相关。【结论】2型糖尿病患者及2型糖尿病合并下肢血管病变者血浆脂联素水平降低,后者尤为重要。 [Objective] To explore the association between the changes of blood plasma adiponectin level in patients with type 2 diabetes complicated with lower extremity arterial disease. [Methods] Type 2 diabetes 58 cases (A group), type 2 diabetes complicated with lower extremity arterial disease 72 cases (B group) and healthy collator 48 cases (C group) were selected. Colored Doppler (CDFI) was conducted on the lower extremity artery for observing the blood vessel inner diameter, the speed of blood flow and the current capacity. Blood plasma adiponectin, fasting blood sugar, insulin, blood lipids were determined. The changes of blood plasma adiponectin level and the pathological changes of lower extremity arterial disease were compared. [Results] Blood plasma adiponectin level of group A was lower than that of group C ( P 〈0.01), blood plasma adiponectin of group B was lower than group A ( P 〈0.01). The speeds of bolld flow, the blood vessel inner diameter and the blood stream quantity in group B were fewer than that in group A ( P 〈 0.01 ), but that in group A were fewer than group C ( P 〈 0.05) ; The blood plasma adiponectin level and lower extremity arterial disease were negatively correlated with FBG, BMI, but positively correlated with HDL. [Conclusions] Plasma adiponectin level is decressed in diabete patient and especially in diabetus patient complicated with lower extremidy arterial disease.
