摘要
目的探讨成人中位硬膜外阻滞初量的使用规律。方法成人中位硬膜外阻滞患者100例,记录初量中的逐次给药量、阻滞节段,计算相对应时点产生阻滞神经范围内的每节段平均需药量(DA/N)以及增加的阻滞神经每节段绝对需药量(V/IB),进行统计分析。结果初量给药次数均在4次以内,DA/N随时点的增加而增加,依次为(0.96±0.41)ml,(1.42±0·48)ml,(1.54±0.38)ml,(1.63±0.53)ml,尤其相邻时点之间有显著的线性正相关(P<0.01),相关方程依次为y=0.89+0.55x,y=0.87+0.42x,y=0.36+0.85x;而不同时点V/IB无明显的规律。结论在成人中位硬膜外阻滞时,初量的分次量可依据不同时点DA/N的均值计算或前次给药时点的相关方程推算。
Objective To explore the law of initiative dose for middle epidural blockade in adults. Methods One hundred patients were scheduled for middle epidural blockade anesthesia. The drug doses and blockade segments of epidural blockade anesthesia were recorded, and the average dose per nerve section (DA/N) and the absolute dose per nerve section within the scope of increased block (V/IB) were calculated and analyzed. Results The initiative doses were given within four times. The DA/N increased with the times [ (0.96 ± 0.41 )ml, (1.42 ± 0.48)ml, (1.54 ± 0.38)ml, (1.63 ± 0.53 )ml, respectively ]. The DA/N showed significantly positive linear correlation (P 〈 0.01 ) between every two adjoining time points, and the equations of correlation were y = 0.89 + 0. 55x, y = 0.87 + 0.42x, y = 0.36 + 0.85x, respectively. The changes of V/IB had no obviously regular pattern at different time points. Conclusion The fractionation dose of initiative dose for middle epidural blockade in adults may be calculated with the aver-age DA/N at different time points or the equation of correlation according to the last dose.
