摘要
目的观察应用Wolter肩锁钩钢板治疗肩锁关节脱位和锁骨远端骨折的疗效。方法2002年4月～2005年10月手术治疗锁骨远端损伤18例,其中应用肩锁钩钢板内固定13例,普通钢板固定3例,张力带固定2例。手术后分析患者的临床治疗效果。结果肩锁钩钢板固定后1周所有患者均能进行肩关节主动活动。平均随访10个月未见钢板断裂和松动,参照Karlsson肩锁关节功能评价标准,肩锁钩钢板固定后A级9例,B级4例,优于普通钢板和张力带固定。结论Wolter肩锁钩钢板固定牢固,能保持肩锁关节的微动,为肩锁关节周围的软组织愈合提供了一个稳定无张力的环境。
Objective To study the surgical treatment of distal clavicle fractures and acromioclavicular joint dislocations using Wolter clavicular plates. Methods From April 2002 to October 2005, the surgical reduction and fixation was performed in 18 patients with distal clavicle fractures and acromioclavicular joint dislocations. Of all the patients, 13 were fixed with Wolter clavicular plates, 3 with ordinary plates and 2 with Kirschner tension band wires. The therapeutic efficacy was analyzed postoperatively and within a mean follow-up period of 10 months. Results In Wolter clavicular plate group,all patients started active shoulder mobilization one week postoperatively, neither acromioclavicular joint redislocation nor breakage or lossening of the plate were found. According to Karlsson＇s standard, in Wolter clavicular plate group, 9 patients were assessed as good, 4 as fair and no poor, it was better than the orther groups. Conclusion Wolter clavicular plate placement is a good choice for unstable distal clavicle injuries, it can maintain the anatomic reduction and micro activity of acromioclavicular joint, and provide a stable condition for the healing of the ligaments and articular capsule.
